Calcitonin. Calcitonin is available commercially from pork and salmon extracts (Calcimar, Armour) as well as by synthesis. Preparations are bioassayed on the basis of their calcium-lowering activity in comparison to the potency of pure pork calcitonin of which ca 4 p.g is equivalent to 1 MRC unit (Medical Research Council, U.K.). For clinical use, vials containing 400 units in 4 mL are available. The recommended daily dosage is 100 units to be adrninistered subcutaneously or intramuscularly because its plasma half-life is short (4—12 min).

Disulfiram is usually given orally. Because there is an increased risk of side effects and toxic hazards as the dosage is increased, the daily dosage prescribed in the United States has been limited to 250—500 mg/day. However, efforts to titrate the dosage of disulfiram in relation to a challenge dose of ethanol indicated that some patients require in excess of 1 g/day of disulfiram to reach blood levels sufficient to produce a DER (Brewer 1984).

Etoposide causes multiple DNA double-strand breaks by inhibiting topoisomerase II. The pharmacokinetics of etoposide are described by a two-compartment model, with an a half-life of 0.5 to 1 hour and a (5 half-life of 3.4 to 8.3 hours. Approximately 30% of the dose is excreted unchanged by the kidney.16 Etoposide has shown activity in the treatment of several types of lymphoma, testicular and lung cancer, retinoblastoma, and carcinoma of unknown primary. The intravenous preparation has limited stability, so final concentrations should be 0.4 mg/mL. Intravenous administration needs to be slow to prevent hypotension. Oral bioavailability is approximately 50%, so oral dosages are approximate two times those of intravenous doses however, relatively low oral daily dosages are used for 1 to 2 weeks. Side effects include mucositis, myelosuppression, alopecia, phlebitis, hypersensitivity reactions, and secondary leukemias.

Oral iron therapy with soluble ferrous iron salts, which are not enteric coated and not slow- or sustained-release, is recommended at a daily dosage of200 mg elemental iron in two or three divided doses (Table 33-3).

Buprenorphine in two formulations (both assigned to schedule III) was recently made available for office-based management of opioid dependence by qualified physicians. Once-daily dosage is titrated to a target of 16 mg/day (range, 4 to 24 mg/day). 

Some chemicals pass into the body with little or no immediate effect, and only produce significant effects years later. One of the most important of these is lead. When you take in small quantities of lead, day by day, by ingestion, or through the skin, you may have little or no problem other than perhaps some mild colic or stomach ache, perhaps not even that if the daily dosage is small. The lead that you take in, however, passes into the bloodstream and is carried throughout the body. For some reason, not clearly understood, it then settles largely in the bones, particularly the long bones such as the arm and the leg, and there it stays year after year as more and more lead is accummulated. 

Cadralazine has been launched under the trade names cadraten and cadri-lan. Pharmacological, pharmacokinetic and clinical studies on it have been reviewed [212]. A once-daily dosage of 15 mg cadralazine has been shown to be effective in the treatment of mild to severe hypertension and to be well tolerated. The therapeutic efficacy and tolerability can be improved by combining cadralazine with a -adrenoceptor antagonist [212].
