Like other steroids, testosterone acts intracellularly in target cells. In skin, prostate, seminal vesicles, and epididymis, it is converted to 5k -dihydrotestosterone by 5K-reductase. In these tissues, dihydrotestosterone is the dominant androgen. The distribution of this enzyme in the fetus is different and has important developmental implications.

Testosterone This agent is ineffective orally because of inactivation by first-pass metabolism. As with the other sex steroids, testosterone is rapidly absorbed by the liver and other tissues, and is metabolized to relatively or completely inactive compounds that are excreted primarily in the urine but also in the feces. Testosterone and its C-17-esters (for example, testosterone cypi-onate or enanthate) are administered intramuscularly. [Note The addition of the esterified lipid makes the hormone more lipid-soluble, thereby increasing its duration of action.] Testosterone and its esters demonstrate a 1 1 relative ratio of androgenic to anabolic activity.

Pituitary reserve is almost never compromised in patients with small pituitary tumors, so testing is usually limited to patients with large tumors or specific clinical indications, (see Box 50-6, A). Both LH and FSH are referred to as gonadotropins, beoause they aot on the male and female gonads, which results in the production of the sex steroids testosterone and estradiol, respectively. In the female, FSH and LH act in concert in regulating ovarian function egg maturation, ovulation, and transformation of the ruptured follicle into the corpus luteum. In males, spermatogenesis is dependent on these two hormones. Specifically, FSH in females facilitates the maturation of ovarian folliole oells and their secretion of estradiol, whereas in males, it stimulates the maturation of sperm in the testes. In females, LH promotes ovulation, formation of the oorpus luteum, and progesterone secretion in males, it enables the secretion of testosterone from the testes.
