Standing instructions

If not already done, set up a titration unit as indicated on page 4 of the Metrohm Model 703 Ti Stand Instructions for Use manual, and an exchange unit as indicated on pages 72 and 73 of the Metrohm Model 701 KF Titrino Instructions for Use manual. Place titrant in a bottle labeled titrant. Place methanol in a bottle labeled solvent. Make sure both bottle caps are tight. Switch on the instrument. 

Standing instructions for dealing with discharges of phosgene have been prepared by the United States Coast Guard [S06]. 

Checklists of important observations required for safe, pollution-free, and efficient operations of the plant which are to be made every half an hour, every two hours, and every four hours shall be clearly written down in standing instruction book. Instructions shall also be available for checking all safety devices while taking charge of the plant from previous shift and while handing over charge to next shift. 

The 61 survivors of the Piper Alpha accident survived because they disobeyed the standing instructions they did not wait for an order to evacuate, they opted for tertiary escape and jumped into the sea where they were rescued by support vessels and fast rescue boats from nearly platforms. 

The staff interviewed operations personnel and reviewed department organization manuals to determine whether all personnel in reactor operations had clearly defined job responsibilities and authority. Sections 3 and 3A of the COM define operator shift responsibilities and shift management responsibilities and authority. Reactor Operations Standing Instruction (ROSI) III.l (Reference 3) provides the organization of reactor operations.

The formal control procedure for experimental activities and tests is DPSOP 336, 11.12 Rev. 3, "Reactor Operation Standing Instruction No. II, 12 Test Control." A section titled "Terms and Definitions" defines various types of tests. For the Startup Test Program (STP) an administrative procedure in the Startup Test Program Manual. STPM-06, "Control of Testing," has been prepared and approved for control of restart testing and defines special tests in support of reactor restart. This meets the restart criteria.

The highest level of administrative controls that outlines the minimum numbers of personnel required to be on shift and within prescribed duty stations is Section 5 of the Technical Specifications. Section 5, the Administrative Controls section of the Technical Specifications, details the minimum number of operations personnel required to safely operate the reactor facility in the various modes (i.e., startup, level operations, shutdown). WSRC shall establish and implement procedures and standing instructions for tagouts. These guidelines shall require a tagout program equivalent to the commercial nuclear industry that implements the requirements of Chapter 9 of INPO 85-017, Guidelines for the Conduct of Operations at Nuclear Power Stations, Revision 1 (Reference 1). 

WSRC, K-Area Standing Instruction D-62, Restart Work Screening, Rev. 2, March 6, 1991. 

K-Area Standing- Instruction D62, Restart Work Screening (Reference 24), has recently been implemented to define and provide directions on processing work requests. The information provided in the instruction supplements the requirements of RD 3.1, Work Control Manual. The instruction provides the routing and review requirements for each work request by priority. This instruction provides the structure and controls which ensure that preplanning and scheduling can take place. 

The RAEP and response EPIPs adequately describe the emergency response functional requirements of personnel. A pre-established policy to bypass job requirements in emergency conditions, provided that safety is not compromised, is addressed in Standing Instruction No. II-8 (Reference 22). This portion of the restart criterion is satisfied.
