Seasonal rhinitis

These agents are an excellent choice for perennial rhinitis and can be useful in seasonal rhinitis, especially if begun in advance of symptoms. Some authorities recommend nasal steroids as initial therapy over antihistamines because of their high degree of efficacy when used properly along with allergen avoidance. 

For seasonal rhinitis, treatment should be initiated just before the start of the offending allergen s season and continue throughout the season.

For seasonal rhinitis, treatment with cromolyn sodium should be initiated just before the usual start of the offending allergen s season and continued throughout the season. In perennial rhinitis, the effects may not be seen for 2 to 4 weeks therefore antihistamines or decongestants may be needed during this initial phase of therapy. As cromolyn sodium begins to work, the need for these medications should decrease.

Dexamethasone (100 mcg/metered spray) is indicated in control of bronchial asthma in patients with steroid-dependent asthma and for relief of symptoms of perennial or seasonal rhinitis and in prevention of recurrence of nasal polyps after surgical removal. 

Histamine is formed from the amino acid histidine and is stored in high concentrations in vesicles in mast cells. Histamine is metabolized by the enzymes monoamine oxidase and diamine oxidase. Excess production of histamine in the body (by. for example, systemic mastocytosis) can be detected by measurement of imidazoleacetic acid (its major metabolite) in tbe urine. Because it is released from mast cells in response to IgE-mediated (immediate) allergic reactions, this autacoid plays an important pathophysiologic role in seasonal rhinitis (hay fever), urticaria, and angioneurotic e ma. Histamine also plays an important physiologic role in the control of acid secretion in the stomach and as a neurotransmitter.

A 22-year-old man with a history of fixed drug eruptions, seasonal rhinitis, and mild atopic dermatitis took one tablet of desloratadine and on the next day developed generalized eczema. Patch tests performed 2 months later were strongly positive and were followed during the next 24 hours by a relapse of the pruriginous eczematous lesions spreading on the trunk.
