Making Health Care Safer H An Updated Critical Analysis of the Evidence for Patient Safety Practices is the result of a panel of patient safety experts who assessed the evidence behind 41 patient safety strategies and identified 10 strategies that health systems should adopt now. The strategies can help prevent harmfnl events snch as med errors, bedsores, and healthcare-associated infections. Making Health Care Safer II npdates Evidence-based Practice Center report ( 43), which was published in 2001 and provided the first systematic assessment of patient safety practices. 

Current health software developments within the Trust are intended to comply with these NHS standards. Developments are supported with a safety case report. The strategy is to build up a set of sub-system safety cases until there is a comprehensive set covering all applicable parts of the Trust s IT. 

The safety case, safety case report and supporting activities were the key to developing a safety management strategy. When the regulations were updated, some parts of the necessary response were aheady in place within the Trust, but not all. 

The STAR-H2 safety strategy is adapted from that used for the IFR, which was demonstrated in full scale tests at the EBR-II sixty-two MW(th) power plant in 1986 tests [XXIV-16] loss of heat sink without scram (LOHSWS) and loss of flow without scram (LOTS) both from full power as well as run beyond cladding breach were all demonstrated to yield benign results. The Level 1 PRA conducted for the EBR-II [XXIV-17] showed that probability of technical specification violation with marginal loss of fuel pin lifetime came in at a slightly lower frequency ( 10 /year) than the probability for core disruption and overall loss of the reactor for the PWR PRAs reported in NUREG-1150, see Figure XXIV-11. 

The Environment Report (Reference 14.4) and Integrated Waste Strategy Report (Reference 14.7) set out radioactive waste management strategies and demonstrate the process chosen are the Best Available Technique (BAT). A brief summary of this BAT assessment is provided in this Pre-Construction Safety Report (PCSR) (Chapter 15). 

The Safe System approach has been fully adopted in countries such as Sweden, the Netherlands, and Australia, and is being pursued in Canada (Road Safety Strategy 2015) and in the United States (Toward Zero strategy). Further, it has been endorsed and recommended to aU countries by the Organization for Economic Cooperation and Development (OECD) in its 2008 report Towards Zero Ambitious Road Safety Targets and the Safe System Approach (OECD/ITF, 2008).
