The Risk Assessment Tool Process—The Risk Guidance Card... 

The matrix used below is a variation on the one used in our book. Job Hazard Analysis (Roughton Crutchfield, 2008). The risk guidance card has been abbreviated as it is to be used to develop an awareness of immediate risk. Refer to Table 9.3 for way for Identifying Probability and Severity . 

To use the risk guidance card (Figures 9.1-9.4) and information in Table 9.3, the user(s) first ask the following questions using the definitions from Table 9.4 and 9.5 ... 

Probabihty and severity are scored on a scale of 1-3. Where the two scores intersect on the matrix is the Risk Rating . Refer to Table 9.6 for Risk Guidance Card-Risk Rating Color Codes .

If you have a risk rating of 3, then the task is stopped, the supervisor is contacted, and the perceived hazard or associated risk situation or condition is discussed. The employee shares the perception of risk with the supervisor and how the assessment was made using the risk guidance card. The supervisor cannot override the concerns of the employee without providing a solution or clearly showing how the hazard and its associated risk can be controlled. Both supervisor and employee must agree with the analysis. 

The risk guidance card is used by employees to conduct a self-assessment before performing a task that may have inherent hazards. The risk guidance card is in the form of a small card carried by all employees. 

The risk guidance card may require a shift in the perceptions held by the leadership team on what its role is in risk management. It requires allowing employees to raise issues without fear of reprisal and with an understanding that disagreement is okay.

To effectively use this checklist, employees on each shift will review the machine or equipment before production begins, rate each checklist item with the risk guidance card, and record the risk rating to determine if the machine/equipment is in acceptable or unacceptable condition to operate. Refer to Chapter 9, Risk Perception—Defining How to Identify Personal Responsibility . 

A member of the central safety committee should be designated to guide and administer the JHA process. This person would use cross-functional ad hoc teams to develop and maintain diversity and expertise for development of a portfolio of JHAs. The JHA ad hoc teams would develop specific JHAs based on the risk assessment list developed using the risk guidance card. They would draw on experience and expertise of employees and supervisors in the department or area where the job resides. Refer to Chapter 8, Getting Your Employees Involved in the Safety Management System .

Note When conducting the daily inspection ofyour machine/equipment, you must perform a risk assessment for each item listed using the risk guidance card.

The risk guidance card can be used to address ongoing employee perceptions or concerns about job hazards.The risk guidance card can be used with a work team and/or employees as part of a prejob assessment. The card provides a Go/No Go decision method as the employee(s) performs assigned tasks (Roughton Crutchfield, 2008,2013).The risk guidance card... 

The intent of the risk guidance card is to allow employees to assess levels of perceived risk, and if believed by the employee to be unacceptable, begin a dialogue with the appropriate leadership level on whether to accept or reject the risk and what controls are to be implemented.This provides a way to reduce the probability that a high risk is accepted without prior discussion with leadership team members. 

For example, when an employee uses the risk guidance card and their perception of the hazard is identified as a Risk Rating of 1, 2, or 3 (RED), then the task is stopped and the leadership team is contacted immediately. 

Given the need for an organization to meet specific production or goals, the pressure to maintain a schedule of activity will consciously or unconsciously pressure employees to see risk as acceptable even when conditions or the situation indicates otherwise (see Figure 8.5). This checklist can be used as part of the one-on-one discussions with employees where the leader and employee will discuss elements on the list and validate that the review is acceptable to both the employee and the leader. If the employee who reviews the checklist finds that an item is unacceptable, based on the assigned risk code using the risk guidance card, the leadership is contacted and a review is conducted tc ether at the machine or equipment. 

Refer to Chapter 8 for a detail discussion on the risk guidance card. If no agreement is reached, the next level of the leadership team is contaaed and a second discussion begins. The intent of having these discussions is to reduce the potential and probability that employees will continue to operate machinery or equipment that is not functioning as intended, believing that they have no alternative but to use the machinery or equipment in an unsafe condition. 

The follow-up team provides an unbiased review of each safety-related work order created, scheduled, and completed within a designated time period or that remains outstanding.The team is chatted with reviewing all safety-related work order, using the risk guidance card to establish a priority for corrective action or moving the request to the next level of decision makers (Chapter 8). 

Percent of hazards assessed for severity using the risk guidance card. Average time between hazard identification and abatement completion. Percent of hazards corrected in a timely manner (timehness is defined). Percent of physical hazards that have been identified, but have not been corrected. 

Provide techniques that allow each individual to assess specific perceived risk and hazard identification with a practical method to prioritize issues. Refer to the risk guidance card as discussed in Chapter 8. 

The safety system is tested by looking at the overall system.
