Pyelonephritis complications

Patients may or may not be in acute distress. In early stages of disease, the patient may complain of obstructive voiding symptoms. If untreated, in late stages of disease the patient may complain of irritative voiding symptoms, or acute urinary retention, which is painful due to maximal distention of the urinary bladder. Also, the patient may be symptomatic of disease complications, including urosepsis, pyelonephritis, cystitis, or overflow urinary incontinence. [Pg.793]

Bacteriuria, or bacteria in the urine, does not always represent infection. For this reason a number of quantitative diagnostic criteria have been created to identify the amount of bacteria in the urine that most likely represents true infection (hence the term significant bacteriuria ). These are shown in Table 76-1. Furthermore, UTIs are classified as lower tract or upper tract disease. Patients will present differently with upper versus lower tract disease, and upper tract disease is thought of as a much more severe infection, as patients are more likely to be admitted to the hospital with upper urinary tract disease than lower tract disease. An example of lower tract infection is cystitis. Cystitis refers to the syndrome associated with a UTI involving dysuria, frequency, urgency, and occasional suprapubic tenderness. An example of upper urinary tract disease is pyelonephritis. Pyelonephritis is an inflammation of the kidney usually due to infection. Frequently, patients with uncomplicated UTI are treated as outpatients, while those patients with complicated UTIs are treated as inpatients. [Pg.1151]

It is known that an association exists between maternal UTI during pregnancy and fetal death, mental retardation, and developmental delay.24 Because of this known association, and because up to 7% of pregnant women have an asymptomatic bacteriuria that may progress to pyelonephritis, screening is necessary. In patients with significant bacteriuria, whether symptomatic or asymptomatic, treatment is recommended to avoid the complications discussed above. In the majority... [Pg.1156]

Levofloxacin (1), the levo-isomer or the (5)-enantiomer of ofloxacin, received FDA approval in 1996 (Fish, 2003 Hurst et al., 2002 Mascaretti, 2003 Norrby, 1999 North et al., 1998). The initial approval covered community-acquired pneumonia, acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, uncomplicated skin and skin structure infections, acute pyelonephritis, and complicated urinary tract infections (North et al., 1998). Four years later, the levofloxacin indication list grew to include community-acquired pneumonia caused by penicillin-resistant Streptococcus pneumoniae. In addition, in 2002, nosocomial (hospital-acquired) pneumonia caused by methicillin-susceptible Staphylococcus aureus, Pseudomonas aeruginosa, Serratia marcescens, Haemophilus influenzae, Kliebsella pneumoniae, and Escherichia coli was added (Hurst et al., 2002). Finally in 2004, LVX was approved as a post-exposure treatment for individuals exposed to Bacillus anthracis, the microbe that causes anthrax, via inhalation (FDA, 2004). [Pg.47]

Lower tract infections include cystitis (bladder), urethritis (urethra), prostatitis (prostate gland), and epididymitis. Upper tract infections (such as pyelonephritis) involve the kidney and are referred to as pyelonephritis. Uncomplicated UTIs are not associated with structural or neurologic abnormalities that may interfere with the normal flow of urine or the voiding mechanism. Complicated UTIs are the result of a predisposing lesion of the urinary tract such as a congenital abnormality or distortion of the urinary tract, a stone, indwelling catheter, prostatic hypertrophy, obstruction, or neurologic deficit that interferes with the normal flow of urine and urinary tract defenses. [Pg.544]

In comparative trials involving commonly used regimens, levofloxacin had equivalent if not greater activity in the treatment of community-acquired pneumonia, acute bacterial exacerbations of chronic bronchitis, acute bacterial sinusitis, acute pyelonephritis, and complicated urinary tract infection (5). [Pg.2048]

An association between the use of 5-ASA in patients with chronic inflammatory bowel disease and the development of a particular type of chronic tubulo-in-terstitial nephritis is difficult to interpret since renal involvement in chronic inflammatory bowel disease may be an extra-intestinal manifestation of the underlying disease [110]. Extra-intestinal manifestations of chronic inflammatory bowel disease are well recognized. The most frequent renal complications are oxalate stones and their consequences such as pyelonephritis, hydronephrosis and on the long-term amyloidosis [111, 112]. As for many drugs, reversible acute interstitial nephritis has been described [90]. [Pg.412]

Urinary tract infections can be classified by anatomic site of involvement into lower and upper urinary tract infections. Lower UTls include cystitis, urethritis, prostatitis, and epididymitis, whereas upper urinary tract infections include pyelonephritis. UTls also may be further classified as complicated or uncomplicated. In females with a structurally normal urinary tract, both cystitis and pyelonephritis are considered uncomplicated UTls. UTls in men, elderly individuals, pregnant women, or patients with in-dwelling... [Pg.117]

Ertapenem is a carbapenem that inhibits cell wall synthesis. It is indicated in the treatment of moderate to severe complicated intra-abdominal infections, complicated skin and skin structure infections, community-acquired pneumonia, complicated urinary tract infections (UTIs) (including pyelonephritis), and acute pelvic infections (including postpartum endomyometritis, septic abortion. [Pg.238]

Two-thirds of the patients with hyperparathyroidism present nephrolithiasis. In the early stages, hypercalcemia leads to hypercalciuria and hyperphospha-turia with polyuria and occasionally polydypsia. Later, calcium phosphate stones form in the kidney, and nephrolithiasis with typical renal colics, hematuria, and a complicating pyelonephritis develop. The stones may reach sizes that make them visible on X-rays and make them responsible for hydronephrosis. Renal... [Pg.352]

Some of the techniques may be limited or inappropriate, depending on the patient s medical condition. Splenomegaly, hepatitis, pneumothorax, rib fracture, osteoporosis, pyelonephritis, thrombotic phenomenon, and recent surgery may preclude the use of thoracic manipulation for fear of further complications. Anemia, pregnancy, and a history of neoplastic processes are relative contraindications. [Pg.591]
