Pulmonary disease embolus

Heart failure, chronic obstructive pulmonary disease, thyrotoxicosis, constrictive pericarditis, ischemic heart disease, sepsis, pulmonary embolus, rheumatic heart disease, hypertension, mitral stenosis, atrial irritation, or complication of coronary bypass or valve replacement surgery. 

The predominant mechanism of AF and atrial flutter is reentry, which is usually associated with organic heart disease that causes atrial distention (e.g., ischemia or infarction, hypertensive heart disease, valvular disorders). Additional associated disorders include acute pulmonary embolus and chronic lung disease, resulting in pulmonary hypertension and cor pulmonale and states of high adrenergic tone such as thyrotoxicosis, alcohol withdrawal, sepsis, or excessive physical exertion. 

Vorinostat (Zolinza) is a histone deacetylase inhibitor that is approved for the treatment of cutaneous T-cell lymphoma in patients with progressive, persistent, or recurrent disease on or after two systemic therapies. The recommended dosing is 400 mg orally once daily. Adverse effects include pulmonary embolus, deep vein thrombosis, thrombocytopenia, anemia, and gastrointestinal disturbances. 

The diffusing capacity will be reduced in aU clinical situations in which there is impairment of gas transfer from the alveoli to capillary blood. Common conditions that reduce the Dlco include lung resection, emphysema (loss of functioning alveolar-capillary units), and interstitial lung disease (thickening of the alveolar-capiUary membrane). Normal PFTs with a reduced Dlco should suggest the possibility of pulmonary vascular disease (e.g., pulmonary embolus) but also can be seen with anemia, early interstitial lung disease, and mUd Pneumocystis carinii (PCP) infection in AIDS patients. 

The signs and symptoms are usually insidious in onset and may consist of anorexia, malaise, edema, anasarca, or ascites, and pericardial and pleural effusions may also be present. As a result of a hypercoagulable state, pulmonary embolism may develop, but rarely results in death. " The incidence of renal vein thrombosis varies from 5% to 62%, " and membranous nephropathy should be suspected when there is a sudden onset of hematuria loin pain pulmonary embolus fluctuating or worsening proteinuria or glomerular filtration rate renal tubular acidosis or an increase in leg edema. Hypertension is found in about 30% of patients and is more common in the presence of renal insufficiency or until the disease is advanced. 

As is the case with most invasive procedures, deep venous thrombosis and pulmonary embolus represent rare but potential complications of the uterine artery embolization procedure [53]. Patients taking oral contraception are known to be at increased risk for venous thromboembolic disease and there may be a transient hypercoagulability after embolization [92-94],... 

One patient developed post-procedural transient ischemia (TIA), but no permanent neurological complications occurred in this series. No deaths were attributable to ERCA although one death occurred within the 30-day perioperative period. Specifically, one patient with advanced HNC (Group III) died from complications of disease progression (pulmonary embolus) on the fourth post-operative day. These complication rates were comparable or lower than those previously reported for endovascular deconstructive techniques and stent-assisted carotid angioplasty of atherosclerotic disease [4, 21, 51]. 

A retrospective study of 616 courses of IVlG in 62 patients (43 male, mean age 58.5 years) with neuropathy found thromboembolic complications in seven patients, five (two myocardial infarction, one acute coronary syndrome, one deep venous thrombosis (DVT) without a pulmonary embolus (PE) and one DVT with PE) of which were deemed early occurring within 14 days of treatment. Daily dose of lVIG>35g was correlated with early thromboembolic complication. Independent risk factors for thromboembolic complications included coronary artery disease and current immobility. Thromboembolic complications were significantly increased in patients with four or more of the risks factors listed in Table 1 [79 ].
