Priority number

The most popular scheme among commercial companies is the assignment of a risk priority number (RPN) based on probabiUty of occurrence, detectabihty, and severity of a particular failure mode. The factors (Occ, Sev, and Det) are each rated on a 1 to 10 scale and then an RPN is based on the product of the three rating values.

For the United States of America, 9/11 was a slap in the face, a punch in the gut (actually, the ultimate sucker punch), and a most serious wake-up call. The 9/11 wake-up call generated several reactions on our part—obviously, protecting ourselves from further attack became (and hopefully still is) priority number one. In light of this important need (i.e., the survival of our way of life), the Department of Homeland Security was created. The weighting helps to determine at which percentage a criteria should be calculated in the total evaluation. The sum of all weighted evaluations results in a priority number which expresses how worthwhile an alternative business model would be (Table 2).

Applying the pseudo-atom convention to the iron complex in Figure 8, the iodine atom is priority 1, the t1S-C5H3R2 ligand is priority number 2, the phosphorus atom is priority number 3, and carbon is priority number 4.15 When the iron is viewed from the side opposite the priority number 4, the sequence is 1,2,3 in the anticlockwise or 5 direction. The highest priority carbon in the cyclopentadienyl ligand, indicated by an asterisk, is designated with the R chirality symbol by application of the extended CIP sequence rule. 

FMEA is a quantitative risk analysis for complex systems (Fig. 6). As this approach involves assessment of occurrence probabilities, detection of failures, and judgment as to the severity of a failure, it should only be chosen if some practical experience with the technical system is available. Each of the three values will be assigned a number from 1 to 5. Multiplying these values results in the risk priority number. This number indicates the priority of the assessed failure. The pure version of the FMEA is seldom practiced in the pharmaceutical industry. 

Variations are often made by cutting the detection of failures or severity of failures. Sometimes the values are decreased to a spread of 1 to 3. In other cases the risk priority number is not calculated, but the levels are noted in a matrix to see whether the point is critical or not.
