When I retired from industry one of my first tasks was to sort the many accident reports I had collected. The thickest folder by far was one labeled Preparation For Maintenance. Some of the incidents from that folder, together with more recent ones, are described above.

In preparing the report for the HSE the company would usually prepare a safety case assessing the nature and degree of the hazard and the consequences of an incident. This would include details of the measures taken to alleviate the hazard and the consequences of an accident. The preparation of safety cases is covered by Lees and Ang (1989). 

A typical system safety task for hazard identification would be the preparation of a preliminary hazard list (PHL). Hazard identification or discovery is accomplished by reviewing lessons learned, accident reports, and other historical data. A PHL may be prepared through an informal conference, the use of checklists, and occasionally other techniques such as energy trace and barrier analysis (ETBA). 

Input data for preparation of the PHL include project description documents (narrative descriptions, sketches, preliminary drawings, artists concepts, drawings or photographs of existing similar products, and analytical trees), historical data (safety, reliability, and loss data and accident reports from similar end products and/or components), and any other relevant studies or research. 

An accident analysis report may be prepared by the system safety working group and/or an independent accident investigation board. The recommended method for serious accidents is for the SSWG to prepare an accident analysis report based on its own investigation and upon the findings of the traditional accident report prepared by the accident investigation board.

Appendix 15.2 provides a checklist of headings, which may assist in the collection of information. It is not expected that all accidents and incidents will be investigated in depth and a dossier with full information prepared. Judgement has to be applied as to which incidents might give rise to a claim and when a full record of information is required. All accident report forms should include the names of all witnesses as a minimum. Where the injury is likely to give rise to lost time, a photograph(s) of the situation should be taken. 

Regulation 7 of the COMAH Regulations requires that top-tier COMAH establishments write a safety report. The safety report must include details of the on-site emergency plan arrangements, and must contain the information required to enable the local authority to write the off-site plan. One of the main sources of the evidence that is available and can be used for risk modeling is accident reports that are prepared by expert accident investigators (Schroder-Hinrichs et al., 2011). Since the accident reports were prepared in a systematic way by expert accident investigators, in order to uniformly extract the knowledge from all the reports, a framework is needed for reviewing the reports. There are a handful of tools and frameworks available for accident and incident analyzing and reporting (Johnson, 2003), which are mostly based on linear or non-hnear accident theories that deal with complex sodo-technical systems. 

Despite the above discussions, the study is bounded with the involved uncertainties. Use of specific databases as AIB and ForeSea to extract the reports may introduce some level of uncertainty that roots to the way that the databases are formulated. Thus, the study needs to be repeated using reports prepared by other authorities and data providers. The main uncertainty in the result, however, comes from the small used datasets as only 22 accident reports are reviewed. In that respect, increasing the number of reviewed reports can increase the validity of such studies.
