Index functional status 30 d after stroke. The results showed that only the severity of neurological deficit predicted greater 30-d mortality in these patients. Patients with hyperthermia on the first day of hospitalization had increased mortality and worse functional status at 30 d, but increased temperature was not an independent predictor of mortality 30 d after PICH. In a study to assess typical early onset complications following ischemic stroke, Weimar et al. (5) looked at a cohort of 3866 patients from 14 neurology departments with an acute stroke unit. In the first week following admission, increased intracranial pressure (ICP) and recurrent cerebral ischemia were the most frequent complications, along with fever, severe hypertension, and pneumonia. Similar concerns are also found in cardiac surgery patients in whom perioperative stroke occurred (6).

It has been suggested that methotrexate increases mortality in patients with rheumatoid arthritis with cardiovascular co-morbidity (11). This assumption was based on a retrospective analysis of 632 patients with rheumatoid arthritis, of whom 73 died. The simultaneous presence of methotrexate and evidence of cardiovascular disease was an independent predictor of mortality. There was no such association with other DMARDs. The authors suggested that this effect may result from a methotrexate-induced increase in serum homocysteine, encouraging atherosclerosis.

Myers et al. (21) studied 6,213 men over a mean follow-up of 6.2 years and determined that peak exercise capacity was the strongest predictor of mortality in both normal subjects and those with established CVD. Among patients with known CVD, every 1-MET increase in exercise capacity conferred a 9% improvement in survival. Patients able to exercise to >10.7 METs were in the highest quintile and demonstrated the best survival.

The strongest predictor of mortality in pulmonary hypertension is RV function. The debate goes on as to the best modality to evaluate the RV function while prospectively following patients. Some of the options include various echo criteria, cardiac MRI, cardiac CT,...

In a study analyzing 219 patients with ruptured aneurysms, Mayfrank et al. (2001) reported increased mortality and unfavourable outcome in patients with additional moderate and severe intraventricular hemorrhage, indicating that severity of intraventricular hemorrhage is an independent predictor of mortality and functional outcome.

Anemia Treatments. Anemia is an independent predictor of mortality in chronic kidney disease patients and is also associated with worsening of cardiovascular morbidity and accelerated rate of kidney damage. The administration of recombinant human erythropoietin (rHuEpo) has greatly reduced anemia in patients with chronic kidney disease.
