Nonpharmacologic Therapy The incidence and severity of bleeding associated with uremia has decreased since dialysis has become the mainstay of treatment for ESRD. Dialysis initiation improves platelet function and reduces bleeding time.42 Improved care of the patient with ESRD, with anemia treatment and improvement in nutritional status, are also likely contributors to decreased uremic bleeding.

Iron deficiency may be present in postgastrectomy patients and occasionally this may mask an underlying megaloblastic anemia. Treatment of the iron deficiency will unmask the megaloblastic process and macrocytes will appear in the peripheral blood. Iron therapy appears able to influence the serum vitamin B12 level and Williams (W6) noted a substantial increase in the serum vitamin level following treatment with oral iron for 3 months. Of his patients, 36 had a low serum vitamin level before iron therapy, and in 26 the level rose to within the normal range. 

Homocystinuria may result from one or several abnormalities in the mechanism whereby homocysteine is methylated to form methionine. About half of the patients respond to treatment with pyridoxine and it is thought that the vitamin overcomes a block at the homocysteine/cystathionine level by mass action (C23). However, Schuh et al. (S22) have recently described a patient who responded to vitamin B12. The infant presented with severe developmental delay, homocystinuria, and a megaloblastic anemia. Treatment with cyanocobalamin was without effect but treatment with hydroxocobalamin resulted in a rapid clinical improvement, and the homocystinuria disappeared. Methionine synthetase activity in cell extracts was normal, while cultured fibroblasts showed an absolute growth requirement for methionine. The defect appeared to be limited to methyleobalamin accumulation and an inability to transfer the methyl group from 5-methyltetrahydrofolate to homocysteine.

Anemia Treatments. Anemia is an independent predictor of mortality in chronic kidney disease patients and is also associated with worsening of cardiovascular morbidity and accelerated rate of kidney damage. The administration of recombinant human erythropoietin (rHuEpo) has greatly reduced anemia in patients with chronic kidney disease. 

Treatment of subacute combined degeneration with masmve doses of citrovorum factor did not prevent neurological deterioration (Ungley, unpublished), and it appears very unlikely that this compound will protect the nervous system in pernicious anemia. Treatment with folic acid or citrovorum factor has no effect on the serum Bw levels of patients with pernicious anemia (Mollin and Ross, 1953b).
