Observation process management review

As the reader has observed in this review, the innovation process is thriving within this new environment. The focus has been on more cost effective polymerisation plants with an improved environmental profile, the replacement of lead stabilisers, scientific studies and support for phthalates, a greater understanding of and an increasing role for impact modifiers, recycling techniques and waste management possibilities. 

The purpose here is to assure that hazards, risks, and safety and health management deficiencies observed in the monitoring and measurement, audit, incident investigation, and corrective and preventive action activities are properly communicated and considered in the continual planning and management review processes.

Control Algorithm Flaws After the ground launch personnel cutbacks, SSLS management did not create a master surveillance plan to define the tasks of the remaining personnel (the formal insight plan was still in draft). In particular, there were no formal processes established to check the validity of the II filter constants or to monitor attitude rates once the flight tape was loaded into the INU at Cape Canaveral Air Station (CCAS) prior to launch. SSLS launch personnel were provided with no documented requirement or procedures to review the data and no references with which to compare the observed data in order to detect anomalies. 

Retrospective reviews of medical records aim to assess the nature, incidence and economic impact of adverse events and to provide some information on their causes. Adverse events are defined as an unintended injury caused by medical management rather than the disease process that results in some definite injury or, at the very least, spent on additional days in hospital (Box 4.1). Definitions are critical in patient safety and one has to be constantly aware of differences in terminology. For instance, a study by Andrews et al. (1997) in the United States showed a 17.7% rate of serious adverse events in a surgical unit, much higher than most other studies. However, their definition of adverse event was different from that usually employed and they used observation rather than record review, as most other studies do. These are not flaws the study is a good one. The point is... 

Customize observation and review. As we have discussed, management, supervisors, and employees must work together to develop meaningful processes for observation and feedback, support, and empowerment, to include activity-based measurements [4].
