A follow-up audit should be conducted after an appropriate period following the implementation. The objectives of this audit are to verify that recommended actions remain in place, and are still working as intended. It also presents an opportunity to review the retention of the lessons learned and to identify any practices, knowledge or awareness items that are being lost. The audit should therefore consider whether the lessons learned were communicated as appropriate throughout the company and to others in the industry. 

The second and third objectives deserve brief elaboration. Until now, most discussions of resistance have taken place in biologically oriented scientific societies, often with a strong disciplinary, national, or regional focus. In the year 1984, the Directive to the European Commission established an industrial accident notification scheme, the MARS, operated and maintained by the Major Accident Hazards Bureau of the Commission s Joint Research Center in Ispra, Italy. The lessons learned from the preclinical qualification of renal safety biomarkers have demonstrated that it is also clear that no single biomarker will be the answer and that a panel approach of novel biomarkers alongside a more intelligent use of currently used biomarkers represents the way forward to inform all stakeholders. This section outlines the processes to evaluate the performance of safety and health management systems and to take corrective action when shortcomings are found. Communications on lessons learned are to be fed back into the planning process. The expectation is that new objectives and action plans will be written in relation to what has been experienced. 

Proposals for a convention on nuclear safety have been made for many years. In 1980, in the aftermath of the Three Mile Island accident, some saw the need for such an instrument. TMI had demonstrated that improvements should be made to the design and operation of what were until then thought to be very safe reactors. The objective would be to ensure that all cotmtries applied the lessons learned from TMI and brought their safety practices up to an acceptable international standard. At that time, however, the proposal for a convention found litde support. 

The lesson learned is to not assume that the leadership team knows what you have accomplished. You need to let them know by regularly communicating what has been accomplished. Ideally, you will be able to provide your accomplishment and feedback in leadership team meetings. Take these meetings seriously and be prepared to quickly and clearly articulate the status of goals and objectives. Use the written objectives in discussions with your supervisors and employees. During conversations, spot check the status of goals that are not directly under your authority but are part of your plan. 

Project Close-down. The objectives of the project close-down process are the planning of the conqrletion of the remaining tasks, project evaluation and the dissolving of the project team Transfer of lessons learned to the permanent organization and to other projects as well as the dissolution of the project environment relationships close the project. Ultimately, the project owner s approval of the project finalizes the project close-down. 

Objective regulations. Safety is achieved by stipulating the top-level objectives which a system needs to accomplish, whilst leading the methodology (i.e. system architecture and process) up to the applicant. These regulations are often based on past experience (i.e. lessons learned). It is a straightforward and familiar concept.
