Lead government agency

The management of terrorist incidents is led by the Home Secretary, who is responsible for the Government s coimter-terrorism response. The Home Office is the lead government agency. The Home Office is responsible for the management of the acute phase of an incident if it is necessary to manage the consequences of an incident, this is the responsibility of the Civil Contingency Secretariat. 

For all 17 accidents of this study, precursors could be identified. None of the 17 accidents could be classified as unforeseen . A total number of 39 precursors were identified in these 17 accidents which seems extremely low compared to normal accident analysis experience. Detailed accident analysis normally retrieves dozens of near misses and deviations leading to the final accident (e.g. van der Schaaf (Schaaf van der, 1992)), implying the existence of many more precursors. However, the limited amount of detailed information present in the FACTS database is the restricting factor here. The FACTS database reflects the kind of accident information companies and government agencies collect. It demonstrates clearly that detailed information about the period before the accident is not often collected. 

The literature is assessed through the end of 1998 and, in some cases, into 1999. We have cited only papers in the peer-reviewed literature or, in a few instances, government agency reports that are readily available. Our approach has been to consider primarily typical examples of major papers in the refereed literature in the relevant areas. In addition, the survey attempted to identify the leading current environmental issues across the whole of business and industry. Opinions were sought from a group of so-called opinion formers, which consisted of pressure groups, journalists, academics and government agencies.
