Inspector General

According to Mary Schiavo, another member of Motley Rice and a former Inspector General of the U.S. Department of Transportation, Railroads are entrusted with vast access and right-of-way in our communities and our nation, but they hold such rights only insofar as they comply with the regulations governing safe operations of both rail and hazardous materials transport. Norfolk Southern and others failed to comply with these laws, regulations, and standards, and failed to put in place known and recommended practices which absolutely would have prevented this deadly crash. ... 

Increased incidents in clinical trials have lead to a recognition of the weaknesses of informed consent procedures. Actually, the FDA has reported that such deficiencies are the poorest area of GCP compliance for more than 12 years (FDA Reports, 2000, Office of Inspector General, 2000a, b).

For the first time in my experience, the issue of intellectual property had become a bone of contention, as had the security of the records themselves. Fred discovered that these two young researchers had commandeered 150 volunteer charts. He recovered them by raiding their offices, and drew up a memo setting limits on such activity. Even so, when the Inspector General later reviewed the program, some 300 charts could not be located. 

In 1975, after reviewing all available documents, the Army IG (Inspector General) team had found nothing to indicate that anyone above the level of the President of the Intelligence Board or the Director of the Medical Research Laboratories at Edgewood had ever approved this proposal. Indeed, when interviewed in 1974, the former Commander of the Intelligence Center testified that he was completely unaware of the plan beforehand. The Intelligence Project Officer and the responsible physician at the Medical Research Laboratories (presumably Dr. Sim) evidently assumed the project had all the authorization it... 

This mission was unlike the usual directive for inquiry or investigation normally assigned to the Inspector General for action.

Friedrich Stromeyer, 1776-1835. German physician, botanist, chemist, and pharmacist. Inspector-general of all the Hanoverian apothecary shops. Discoverer of the element cadmium. His collection of thirty mineral analyses is a classic of analytical chemistry. 

In 1802 he became a Privatdozent in the faculty of medicine at Gottingen, and was rapidly promoted until in 1810 he became a full professor (Professor ordinarius). In the German universities, as in certain American ones, professors frequently hold government offices. Dr. Stromeyer was the inspector-general of all the apothecaries of Hanover. On an inspection trip to Hildesheim in the autumn of 1817 he noticed that a certain preparation which, according to the Hanoverian Pharmacopoeia, ought to have contained zinc oxide, contained zinc carbonate instead. The events which followed were described by Dr. Stromeyer in his letter to Dr. J. S. C. Schweigger written on April 26, 1818 ... 

Alexandre-Emile Beguyer de Chan-courtois, 1820-1886. Inspector-general of mines and professor of geology at the Ecole Sup neure des Mines in Paris He made geological explorations m France, Asia Minor, Iceland, and Greenland. Technically, Team Biologies consists of two distinct entities the core group and the cadre. The cadre—consisting of 130 inspectors exclusively concerned with blood and plasma—reports directly to ORA field offices. It was created in response to an Inspector General s audit and functions independently of the core group. The term Team Biologies actually refers to the core group.

DSHEA severely limited when the FDA could take action to protect the public and what actions could be taken. The burden of proof to show harm is now placed on the FDA. Moreover, dietary supplement manufacturers are not required to report adverse dietary supplement events. In fact, between 1994 and 1999 fewer than 10 of the 2500 adverse events associated with dietary supplements and reported to the FDA were reported by the manufacturer (53). The Office of Inspector General concluded the spontaneous adverse event reporting system has difficulty generating signals of possible public health concern due to limited medical information, product information, manufacturer information, consumer information, and ability to analyze trends (57). One weight loss supplement manufacturer is reported to have withheld from the FDA 14,684 complaints of adverse events regarding ephedra, which included heart attacks, strokes, seizures, and deaths (53). 

Interior Department Inspector General Earl Devaney reported that he found no criminal intent in the employees actions, and the Justice Department declined to prosecute them. However, he said the USFWS failure to administer meaningful punishment showed the service s bias against holding] employees accountable for their behavior. 9...

The preventive and evaluative tasks that the inspectors have, which in a Swedish context are defined as promoting, counselling or supportive enforcing, will not be further discussed in this article. It is, however, important to keep in mind that Swedish inspectors generally have both supportive and evaluative tasks, even if the occupational title emphasises the control functions. 

To assess the physicochemical processes in Nefertari s tomb, to identify the mechanisms of its deterioration, and to propose a feasible scheme for its conservation and restoration, the Inspector General of the EAO, Hishmat Messiha, urged our group to begin investigations. A permit to obtain representative samples from the tomb and its environment was issued to us by the EAO in 1980, and subsequently, our work was initiated (28).
