It is clear that patients with febrile neutropenia represent a heterogeneous group. Some patients are at lower risk and potentially could be treated as outpatients, thereby avoiding the risk and cost of hospitalization. The Multinational Association for Supportive Care in Cancer (MASCC) has validated a risk-assessment tool that assigns a risk score to patients presenting with febrile neutropenia7 (Table 96-3). Patients with a risk-index score of 21 or greater are identified as low risk and are candidates for outpatient therapy (discussed under Treatment ). 

Diffuse large B-cell lymphomas are the most common lymphoma in patients of all ages but most commonly seen in the seventh decade. Extranodal disease is present at diagnosis in 30% to 40% of patients. The International Prognostic Index score correlates with prognosis. Diffuse aggressive lymphomas are sensitive to chemotherapy with cure achieved in some patients. 

A survey of the general Odour Annoyance Index scores obtained over the different weeks in Hoogvliet and Zeist is given in figure 1. 

Category III prostatitis/chronic pelvic pain syndrome. A recent study evaluated the use of AT clearance as a marker of clinical relapse in patients with Crohn s disease and showed a negative predictive value of 94% for predicting relapse in the next 6 months (at a cut-off of 120 mL/day). Sensitivity was only 50% and comparable results were noted when the Bristol Simple Index score was used to predict relapse so tliat at present the test appears to have Little clinical value in this context ... 

HRQOL as assessed by preference-based measures is a single overall index score on a scale anchored by 1.0 (full health) and 0.0 (dead). Health states considered worse than dead can be reflected by negative numbers on the scale. This approach combines the measurement of an individual s health status with an adjustment for the relative desirability of or preference for that health state. The preferences are measured or assigned empirically through a variety of procedures. Although often called health state utilities, the term preferences will be used in this chapter as the broader term because it subsumes both utilities and values. ... 

Linear regression analyses indicated that the psychomotor index scores declined with increasing prenatal PCB exposure at both 6 and 12 months. At 6 months, the PDI was estimated to decrease 0.96 points for every increase of 1 ppm in PCBs. This would mean a drop of 2.6 points if a child moved from the 5th to the 95th percentile of PCB exposure. At 12 months, the drop was estimated at 1.34 points/ppm. Neither the 6-month nor the 12-month mental index scores were related to transplacental PCB exposure (Gladen et al. 1988). Similar analyses were run to examine postnatal exposure in breast-fed children. Postnatal exposure to PCBs was not associated with the PDI or MDI scores at either time period.

The sample in the main Seychelles study appears to have been developmentally robust. There was an exceptionally low number of abnormal scores on the Denver Developmental Screening Test, an unusually high mean Psychomotor Development Index score, and a very low rate of referral for mental retardation. On the other hand, the means and standard deviations of the cognitive measures administered at later ages were similar to U.S. norms. It is unclear to what extent the developmental robustness of that particular sample might have buffered it from any adverse effects of prenatal Hg exposure.
