A follow-up study has been carried out in 105 children with collapse (a hypotonic-hyporesponsive episode or a shock-like syndrome) after their first immunization with DTwP -I- IPV vaccine (11). Information about subsequent immunizations, health, and development in 101 of the children was supplied by child health-care units. The parents of one child refused further immunization, 16 children completed their schedule with the combination diphtheria -I- tetanus -I- poliomyelitis vaccine (DT-IPV), and the other 84 children received further pertussis vaccine (DTP-IPV), totalling 236 doses 74 children received the complete series of three additional doses. None of the children had recurrent collapse, and other adverse events were only minor. About half were given paracetamol prophylactically for the first subsequent dose most of them did not take it for further doses. The authors suggested that it is unnecessary to withhold further doses of pertussis vaccine in a child with collapse after a previons dose. It has been suggested that the threat of natural pertussis in non-immunized children should be taken much more into account than the fear of developing a collapse reaction (12). In another study (13) in the USA, one of the 14 children not completely immunized becanse of a hypotonic-hyporesponsive episode after a previons dose later developed natural pertussis, which lasted for 3 months and was transmitted to both her parents. 

In the health sciences field, preventive medicine has traditionally been defined as the ensemble of actions and advice aimed specifically at preventing diseases " and promoting health, and the process for enabling individuals and communities to increase their control over the determining factors involved in health to improve itJ Besides health-restoring activities, there are also those that are fundamentally preventive such as immunization, health education for healthy people, and screenings. 

In any case, I eventually recovered (and so did Jerry), but my immune system must have suffered serious damage, which manifested itself three years later, when I collapsed in my office one day and was found to be bleeding internally from a form of rare stomach cancer, which necessitated major surgery but was fortunately localized. I again recovered and have had no further difficulties since. Whether weakening and knocking out my immune system to overcome the previous problems had any effect is not clear, but it could have been a factor. Research on an hCG vaccine has been conducted over the past 15 years. WHO has conducted a phase I clinical study in AustraUa, using a vaccine based on a synthetic C-terminal peptide (109—141) of P-hCG conjugated to Diptheria Toxoid (CTP-DT), that showed potentially effective contraceptive levels of antibodies were produced in vaccinated women without any adverse side effects. Phase II clinical studies are under consideration to determine if the immune response, raised to its prototype anti-hCG vaccine, is capable of preventing pregnancy in fertile women volunteers (115). While research on the C-terminal peptide from the P-subunit of hCG has been carried out under the auspices of WHO, research supported by the Population Council and the National Institutes of Health has involved two alternative vaccine candidates (109,116,118).

While it is true that in the absence of external fuses it is not possible to assess the health of a unit, it is equally true that an intact external fuse is also no guarantee of a healthy unit. Tuberculosis is a major health problem throughout the world, infecting more than 8 million individuals each year. It is die world s leading cause of death from infectious disease Individuals living in crowded conditions, those widi compromised immune systems, and individuals widi debilitative conditions are especially susceptible to tuberculosis. 

However, some health care providers may not prescribe an antipyretic for the patient with an elevated temperature because evidence suggests that fever activates the immune system to produce disease-fighting antibodies. The decision to treat an elevated temperature with an antipyretic is an individual one, based on the cause of the fever and the patient s physical condition. 

Lentinan, a derivative of the shiitake mushroom, is proving to be valuable in boosting the body s immune s/stem and may prolong the survival time of patients with cancer by supporting immunity. In Japan, lentinan is commonly used to treat cancer. Additional possible benefits of this herb are to lower cholesterol levels by increasing the rate at which cholesterol is excreted from the body. Under no circumstances should shiitake or lentinan be used for cancer or any serious illness without consulting a primary health care provider. 

On occasion, it may be necessary to postpone the regular immunization schedule, particularly for children. This is of special concern to parents. The decision to delay immunization because of illness or for other reasons must be discussed with the primary health care provider. However, the decision to administer or delay vaccination because of febrile illness (illness causing an elevated temperature) depends on the severity of the symptoms and the specific disorder. In general, all vaccines can be administered to those with minor illness, such as a cold virus and to those with a low-grade fever. General interventions, such as increasing the fluids in the diet, allowing for adequate rest, and keeping the atmosphere quiet and nonstimulating, may be beneficial. The primary health care provider may prescribe acetaminophen, every 4 hours, to control these reactions. Local irritation at the injection site may be treated with warm or cool compresses, depending on the patient s preference. A lump may be palpated at the injection site after a diphtheria, pertussis, tetanus (DPT) injection or other immunization. This is not abnormal and will resolve itself within several days to several months. 

The Vaccine Adverse Event Reporting System (VAERS) is a national vaccine safety surveillance program co-sponsored by the Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration (FDA). VAERS collects and analyzes information from reports of adverse reactions after immunization. Anyone can report to VAERS, and reports are sent in by vaccine manufacturers, health care providers, and vaccine recipients and their parents or guardians. An example of the VAERS and instructions for completing the form are found in Appendix F. Any clinically significant adverse event that occurs after the administration of any vaccine should be reported. Individuals are encouraged to provide the information on the form even if the individual is uncertain if the event was related to the... 

Educating the Patient and Family Because of the effectiveness of various types of vaccines in the prevention of disease, nurses must inform the public about the advantages of immunization. Barents are encouraged to have infants and young children receive the immunizations suggested by the primary health care provider. 

The nurse advises those traveling to a foreign country to contact their primary health care provider or local health department well in advance of their departure date for information about the immunizations that will be needed. Immunizations should be given well in advance of departure because it may take several weeks to produce adequate immunity. 

Hepatitis B Hepatitis B vaccines provide another illustration of how drug products have advanced vdth increasing technical capability. Vaccination against hepatitis B is common pradice for health workers, travellers and others who may be at risk of exposure to the virus. The initial vaccines contained inactivated virus to promote the immune response necessary to proted against future infection by the live virus. However, there was always some concern in case there was not complete inactivation of the virus used for vaccination.
