Musculoskeletal disorders (NMD and TRD) are relatively uncommon, but when present are often complicated by respiratory failure (20). Data on the burden of patients with CRF from musculoskeletal disorders are quite heterogeneous. Of the 79 patients who received HMV for CRF in a rehabilitation facility in California, 64% had poliomyelitis with bulbar and respiratory involvement (21), the next most common being Duchenne s muscular dystrophy. Other studies have reported CRF in scoliosis, kyphosis, and post-thoracoplasty TRD, as well as in myopathies such as Ducheime s muscular dystrophy, myotonic dystrophy, postpoliomyelitis, and amyotrophic lateral sclerosis (22). [Pg.33]

The effects of treatments with HMV on survival have been poorly studied and the benefits of HMV over LTOT alone, or the benefits of combining both, remain unanswered (20,22). Oxygen alone is associated with poor outcomes (23). Reports on patients with muscular dystrophy in England and Wales have confirmed that even though the number of deaths per annum remains unchanged, the median survival has increased (22). Most deaths (59%) were due directly to respiratoiy failure, but respiratory failure may also have been a [Pg.33]

COPD is one of the major leading causes of morbidity and mortality worldwide and is projected to move from the sixth to the third most common cause of death (24). In addition, COPD will also move from fourth to third place in the morbidity chart by 2020. Disease prevalence will continue to increase in both the developed and the developing world (24,25). It is possible that new options for management of tbe primary disease as well as the comorbid conditions may improve survival and quality of life. [Pg.34]

CRF occurs in tbe very advanced stages of stable COPD when the degree of airflow obstruction becomes severe (26,27). In a report of hospital discharges, respiratory failure accounted for 37% of in-hospital mortality in patients with COPD, as either a primary or a secondary discharge diagnosis (28). Treatment of respiratory failure accounts for a large part of the economic burden of tbe disease. Admission to the hospital for COPD exacerbations, associated with respiratory failure, accounts for approximately two-thirds of the direct costs of COPD (29). The role of HMV in COPD is discussed elsewhere in this text. [Pg.34]

CF is one of the most common lethal genetic diseases in the Caucasian population, affecting approximately 1 in 3400 live births (31,32). Over 85% of patients with CF die from progression of their lung disease. CF has been reported as an uncommon cause of CRF in adulthood (Table 5), although the pattern of the disease is changing as patient life [Pg.34]


The primary uses of pyridoxine hydrochloride are in multivitamin supplement tablets and for fortification of human food and animal feed, especially for poultry and pigs. Most breakfast cereals and infant formulas in the United States are supplemented. Lesser amounts are used therapeutically to correct deficiencies or to treat specific disorders. Pyridoxine hydrochloride has been used experimentally to treat a variety of conditions with varying degrees of effectiveness (4,23). Pyridoxine hydrochloride is readily incorporated into premixes and foods. [Pg.71]

Diuretics are used in a variety of medical disorders. The primary health care provider selects the type of diuretic diat will most likely be effective for treatment of a specific disorder. In some instances, hypertension may be treated with the administration of an antihypertensive drug and a diuretic. The diuretics used for this combination tiierapy include the loop diuretics and the thiazides and related diuretics. On occasion, it may be necessary to postpone the regular immunization schedule, particularly for children. This is of special concern to parents. The decision to delay immunization because of illness or for other reasons must be discussed with the primary health care provider. However, the decision to administer or delay vaccination because of febrile illness (illness causing an elevated temperature) depends on the severity of the symptoms and the specific disorder. In general, all vaccines can be administered to those with minor illness, such as a cold virus and to those with a low-grade fever.
