The developments are assumed to have some effect (positive or negative) on the risk the effect working through a set of risk influencingfactors (RIFs - including technical, human and organisational factors) in a risk influence model. A RIF is a set of relatively stable conditions influencing the risk that may be improved or worsened by specific actions.

As seen with the OMT-RIFs, they are very much the same. The recommendations and the result are interesting for identifying requirements for the critical steps. Typically standard QRA-studies are mainly focusing on the technical factors, but in an evacuation situation the human and organisational factors are of high importance.

Abstract Although this book concentrates on the safety of complex systems, it seemed to me that it would be useful to finish with a presentation of the major historical discontinuities and the successive principles that have guided human and organisational factor (HOF) processes in enterprises. 

These fundamental features of approaches to human and organisational factors underwent little further change until the 1960s. Knowledge of ergonomics and working conditions increased, but there was no revolutionary change.

Human beings are prone to fail due to limitations in perception of risk and their ability to interpret and act on information. There is a variety of ways in which human failure may manifest itself in the workplace. On some occasions people may omit owning up to something they should have done, or taking an inappropriate course of action.
