In terms of the number of consultations made with local physicians, the respiratory health of children living in communities close to opencast sites was very similar to that of children living in communities distant from such sites (Pless-Mulloli et al. 2001 Howel et al. 2001). Nevertheless, in four-fifths of the cases studied, there was a small but statistically significant increase in consultations for respiratory, skin and eye conditions in those living close to opencast sites, albeit the absolute numbers of consultations were still small. Overall, the evidence gathered to date supports the contention that such increments in exposure to PM]0 as may arise in local communities on account of opencast coal extraction are highly unlikely to have any detectable effects on health (Pless-Mulloli et al. 2000a, b, 2001 Howel et al. 2001). 

It is known, however, that drugs are readily available in many prisons, and the rate of adverse incidents and the time and effort spent in detecting smuggling of drugs in has been enough to persuade some authorities that at least the basics of treatment should be available. The most routine option has become to provide a detoxification for opiate misusers, with for instance lofexidine or dihydrocodeine, and also benzodiazepines will often be issued if there is a history of abuse of these and it is intended to avoid the possibility of fits with a short withdrawal course. The adverse incidents in custody and prisons have included some deaths in users of crack cocaine, with physical explanations postulated but no very satisfactory treatment for cocaine withdrawal indicated. Prison services have typically been wary of methadone, and in favouring lofexidine use it was encouraging that a randomized double-blind trial carried out by prison specialists found lofexidine to be as effective as methadone in relief of withdrawal symptoms (Howells et al. 2002).

That CT enterography helps detect both active Crohn disease and small howel strictures is particularly important now that early reports have shown that endoscopic patency capsules may themselves precipitate small bowel obstruction (Gay et al. 2005 Boivin et al. 2004). Any system designed to detect colonic polyps may potentially detect polypoid objects that have soft-tissue CT density and protrude into the lumen. Therefore, any polypoid soft-tissue structure in the small bowel, stomach, or even the lungs can he erroneously detected as a polyp. Thus, the first step of CAD after reading in the CTC images is the extraction of the colonic wall while removing other luminal organs with soft-tissue density, such as the small howel, stomach and lung bases.

The volatile material to be estimated is placed in the outer annular compartment, the absorbent being in the inner compartment, where it is titrated subsequently without removal from the unit.
