Tetracyclines hormonal

Certain antibiotics such as the tetracyclines, streptomycin, neomycin and kanamycin can cripple the tubules if taken in excessive amounts. Hormone production in patients with black thyroid, who had taken a tetracycline for prolonged periods, was normal (1106,1107). 

Several systems have been established which can regulate gene expression. For instance, there are systems which rely on the addition of steroid hormones or heavy metal ions. However, certain physiologic or toxic effects may result and high basal-transcriptional activity may limit their usefulness (Furth et al., 1994). Another approach has been developed which is based on the tetracycline-resistance operon tet from E. coli transposon Tn70. 

Allergic reaction due to streptokinase overdose should be treated with corticosteroids and histamines. Severe hemorrhage requires discontinuation of streptokinase. Packed red blood cells are preferable for blood-replacement therapy, and volume expansion is advisable. Streptokinase may be used with caution with heparin, allopurinol, sex hormones, sulfonamides, tetracyclines, and dextran. 

When acne is severe it may require medidnes that have to be taken by mouth and these may be stronger antibiotics combined with hormone therapy. Tetracyclines and erythromycin are typically used and they have to be taken for many weeks, but there are signs that bacteria are becoming resistant to these antibiotics and if there is no improvement in the acne within three months then this type of treatment has to be discontinued, or a much stronger antibiotic tried. Hormone therapy is available for young women with severe acne and this can block the sebum-stimulating effect of natural hormones - as well as having a contraceptive effect. 

An unexpected use for a tetracycline is in the treatment of chronic hyponatraemia due to the syndrome of inappropriate antidiuretic hormone secretion (SIADH) when water restriction has failed. Demeclocycline produces a state of unresponsiveness to ADH, probably by inhibiting the formation and action of cyclic AMP in the renal tubule. It is effective and convenient to use in SIADH because this action is both dose-dependent and reversible. 

Contraceptive efficacy can be decreased when taken concurrently with drugs that increase the metabolism of hormones (e.g., CYP450 inducers). Similarly, drugs that reduce enterohepatic recycling of COCs (e.g., tetracycline, penicillins) can also reduce efficacy.
