The primary responsibilities of CMS, formerly known as Health Care Financing Administration (HCFA), include administration of Medicare and Medicaid programs. Medicare is the health insurance program for people 65 years of age or older, certain younger individuals with disabilities, and citizens with End-Stage Renal Disease (permanent kidney failure with dialysis or a transplant, sometimes called ESRD). Several different types of billing mechanisms have been used to gain compensation from third parties for services that are not tied directly to dispensing a drug product. Examples include fee-for-service, capitation payment, and the Health Care Financing Administration (HCFA) 1500 claim form. Each method has inherent advantages and disadvantages and may not be a suitable method for compensation for a comprehensive pharmaceutical care practice. 

Both pharmacists and pharmacies use Health Care Finance Administration (HCFA) Form 855, Provider/ Supplier Enrollment Application, to request a provider number. This form may be obtained from medicare part B carriers or provided by the previously mentioned training programs. This is a cursory review of medicare policies. ASHP, in cooperation with its professional association partners, administers the only process that grants accreditation status to practice sites conducting residencies. ASHP s authority to grant accreditation is recognized by the Health Care Financing Administration (HCFA). The accreditation process requires that each site demonstrate compliance with established standards of practice and offer a residency that meets the requirements for training. 

Healthcare providers (physicians, hospitals, clinics, etc.) operate and are sustained by the reimbursement or payment for care they provide to patients. In the United States, revenue for healthcare services comes from a variety of sources including the patient, insurance companies (i.e., Blue-Cross/BlueShield, Cigna, Aetna), and the Centers for Medicare and Medicaid Services (CMS), formerly the Health Care Financing Administration (HCFA). The CMS provides healthcare insurance for Medicare and Medicaid beneficiaries. When interfacing with an insurance company or the CMS, there are three fundamental reimbursement concepts that must be considered to assure appropriate revenue for services provided. These include the concepts of coverage, coding, and payment. 

By law, Medicare does not cover any drugs administered outside of the hospital or a physician s office, and the program does not pay for clinical research (487). Furthermore, to be covered by Medicare, drugs must be reasonable and necessary, a criterion that the Health Care Financing Administration (HCFA) has interpreted. .. to exclude. .. those medical and health care services that are not demonstrated to be safe and effective by clinical evidence (487). HCFA has taken this to mean that experimental and investigational drugs are not covered.

Laboratory inspection programs assure that the laboratory maintains up-to-date documentation, procedure manuals, qualified personnel, and properly maintained laboratory equipment. Successful laboratory inspections are required to maintain accreditation by the Joint Commission on Health Care Organizations and the HCFA.
