Provider number

Instruction signs should be posted at all emergency systems in which the operation of the unit is not inherently obvious (i.e., fire pump startup, fixed foam systems, etc.). Flow arrows of water flow should be provide on piping where the isolation means is provided. Number of hydrants, monitors, pumps, foam chambers can all enhance the operational use of such equipment. Control panel labels should be provided which are descriptive instead of just numerical identifications. [Pg.243]

The sterilization process is completely controlled and monitored by the automatic control unit and registered by the (provide numbers) channels (provide manufacturer name) recorder. [Pg.504]

Autoclave. (ABC 009) serial no. (provide number) autoclave is utilized for sterilization of solution buffer vessels, filters, filter housings, closures, filling machine dosing system, and support equipment. The unit has a chamber size of (width x length x height) mm and (provide volume) liter capacity with dual interlocking doors. [Pg.506]

Slowest allowable line operating speed for a particular hll machine to ensure maximum exposure of unsealed vial or fastest allowable line operating speed to ensure maximum manipulations of vials. Line speeds depend on the machine and vial size. Typical line speeds range from (provide number) vials per minute to (provide number) vials per minute. [Pg.515]

For change control required and process deviation control, refer to SOPs (provide number) and (provide number), respectively. This may be postponed subject to the management decision as appropriate and justified. [Pg.544]

Particulate quantitative air sampling will be performed using device CI-500 innovation laser particle counter (serial no. mentioned on the monitoring format) according to SOP (provide number)... [Pg.693]

QAI will check the particulate of the manufacturing area using laser particulate counter, per plant SOP (provide number). [Pg.703]

QAI is responsible to give the line clearance and line release according to SOP (provide number) and countersign the formats. See attachment no. 1700.30(D) and 1700.30(E). [Pg.703]

When filling of the ampoules starts in the area, QAI will be informed. He will check the area/equipment clearance and then check the dose of the filled ampoules (by weight and volume), length of the ampoules, and sealing and also check that ampoules/vials are free from particles. See SOP (provide number). When the dose of the filled ampoules is found satisfactory, QAI will give release for filling... [Pg.703]

After completion of visual inspection by manufacturing, QAI will perform (a) the review of printout of cold leak test carried out by production and (b) the visual inspection. See SOP (provide number). He gives the release for packing if the results are satisfactory. QAI will fill the optical inspection report attachment no. 1700.30(G) and 1700.30(H) for each batch optically inspected and for leakage. When packaging of the batch starts, the production supervisor will request the area QAI for line clearance. The QAI will check the area and countersign the line clearance given by the supervisor. See attachment no. 1700.30(B). [Pg.704]

Refer to attached drawing (provide number), it is appropriate to increase or decrease sampling based on the trend performance. The time and date shall be determined according to the activity. [Pg.739]

Aseptic techniques used should be reviewed periodically to ensure that departures from aseptic practices do not develop. Personnel should undergo periodic aseptic technique training (SOP [provide number]), particularly when problems are detected (during the course of routine environmental and negative control monitoring) or when operators perform the test infrequently. [Pg.812]

Note 1 Maximum allowable reject rate is 2%. Investigate if rejects exceed this limit. If the limit is exceeded, refer to SOP no. (provide number) Note 2 For samples of defects, see the photos in attachment no. 1700.110(D). [Pg.822]

Airborne bioburden will be analyzed per SOP (provide number). Airborne particulate will be analyzed per SOP (provide number). Surface bioburden will be analyzed per SOP (provide number). In addition, surface samples of the room will be analyzed per the weekly requirements of this SOP. [Pg.874]

Monitoring the environment for viable and nonviable particles shall be conducted during each media fill per SOP (provide number) the same as if it were a normal production run. [Pg.905]

Water used will be monitored per SOP (provide number). [Pg.906]

Growth promotion shall be performed per standard test method (provide number), where USP growth promotion test organisms and representative panel of microorganisms normally found environmentally or in the product bioburden shall be used in growth testing the medium-filled containers. [Pg.908]

Dr. Brouchard develops a referral form for his service and has included fields for the physician s office to complete regarding the type ofinsurance, the policy number, the ICD-9 codes, and the physician s provider number. He plans on requesting that referring physicians complete a referral form for each patient they send to the service. If a patient approaches him directly for the service, he plans on contacting his or her physician... [Pg.463]

Contrary to viscosity measurements GPC provides number average molar mass data (Mn). For a few studies on the polymerization of BD with Nd-carboxylate-based catalyst systems GPC was systematically applied for the monitoring of Mn as a function of monomer conversion. In these studies three catalyst systems were used (1) NdV/DIBAH/EASC [178], (2) NdV/TIBA/EASC [179] and (3) NdO/TIBA/DEAC [188]. In the first two studies linear Mn-conversion plots were obtained at various molar ratios Al/ Ndv- In these studies, however, molar mass data at low monomer conversions (<20%) are lacking and positive intercepts on the Mn-axis were found. For the ternary catalyst system NdO/TIBA/DEAC used in the third of these studies the concentrations of Nd and TIBA were varied. A linear increase of Mw and Mn on monomer conversion was found. Deviations from linearity were also observed for low monomer conversions. [Pg.119]
