In a randomized trial a single oral dose of ivermectin (200 micrograms/kg) has been compared with 1% gamma-benzene hexachloride lotion for topical application overnight in 200 patients with scabies (16). The patients were assessed after 48 hours, 2 weeks, and 4 weeks. After 4 weeks, 83% showed marked improvement with ivermectin, compared with 44% of those treated with gamma-benzene hexachloride. There were no adverse events reported with gamma-benzene hexachloride. Headache was reported only once with ivermectin. 

In a randomized, double-blind comparison of the efficacy of oral ivermectin and topical gamma-benzene hexachlor-ide 53 patients were randomly allocated to either a single oral dose of ivermectin 150-200 micrograms/kg and a placebo topical solution, or a single dose of gamma-benzene hexachloride topical solution 1% and placebo tablets (22). Patients who did not fulfil the criteria for clinical cure within 15 days, defined as the absence of both pruritus and clinical lesions or a reduction in signs... 

In 1976, the US Food and Drug Administration (FDA) published a Gamma Benzene Hexachloride (Kwell) Alert, based in part on several poorly documented cases of convulsions after topical treatment with lindane (6). Indeed, several authors have reported convulsions (7-9), but in most cases lindane had been inappropriately used (4,7), while another case took place in unusual therapeutic circumstances (prematurity, marasmus, pneumonia, congestive heart failure, ventricular septal defect) (8). A child with convulsions after the application of lindane had tuberous sclerosis and may therefore have had a reduced threshold for convulsions (9).

Lindane (gamma benzene hexachloride) is used to treat infestations with mites or lice and also as an agricultural insecticide. The agent can be absorbed through the skin if excessive amounts are applied, toxic effects, including blood dyscrasias and convulsions, may occur.

When included with another insecticide such as gamma benzene hexa-chloride, dieldrin or chlorparacide, DDT is usually present at a concentration several times greater than that of the other material. Therefore, to make simultaneous infra-red measurements of the two insecticides on the same extract, a weaker absorption band than that at 1,015 cm must be used. The band at 896 cm satisfies this demand and is also in close proximity to the bands at 913 cm , 912 cm and 883 cm which may be used for measurements on dieldrin, gamma benzene hexachloride and chlorparacide respectively. The concentration of DDT in the final solution is usually around 4 per cent and to effect solution carbon tetrachloride... 

Gamma benzene hexachloride is readily decomposed with hot alcoholic alkali to give the trichloro-compound and determination of the total hexa-chlororyc/ohexane is based on this fact. 

Polarograph this solution in triplicate using a mercury pool anode over the range -0 2 to —1 7 volts maintaining the solution at a fixed temperature of about 25°. Polarograph also under the same conditions a solution containing 13 mg of gamma benzene hexachloride in 100 ml prepared as above.
