In an organization that has not historically evaluated performance using objective data, the initial reported results may appear to indicate that performance has declined. This sometimes occurs when the historical perspective is too optimistic. For example, the number of near misses often increases as reliability of reporting improves. The recipients for metrics reports should be advised what to expect and provided explanation as to why the results may not meet expectations. The potential cultural impact, if left unaddressed, may raise doubt about the accuracy and value of the metrics reports, and these potential negative reactions need to be managed.

The HSEQ Assessment Procedure is a practical tool developed to evaluate health, safety, environment and quality management in the supply network of the process and manufacturing industry (Vayrynen et al. 2012). It offers companies an evaluation performed by an outsider and thus, a bias-free reference for companies in the process of comparing and choosing suppliers. These suppliers also have an oppoitunity to ask for an HSEQ Assessment Procedure (later referred to as HSEQ AP) to be carried out for their network s suppliers. 

Therefore it is clear that the quality of information and the stage when it is available also influence PHA method selection. For the same reason it is not possible to carry out HAZOP for a plant at the conceptual stage. So, if quality data are not made available, then the team and the team leader have probably only two options they can either recommend to the authority to change the detailing desired from PHA or delay the selection and inform management. In the last example we discussed the lack of information. Similar things happen if the available data are not up to date. It is wrong to work with old data because such an exercise is futile. Therefore it is essential that the team leader is aware of this so that hazard evaluation performance is improved and not delayed.
