Errors and omissions

The list of contributors is on page 6 and I am deeply indebted to them, for it is they who originally prepared MiaU s Dictionary of Chemistry from which this Dictionary has been compiled. Errors and omissions are my responsibility and 1 would appreciate receiving notice of them. 

The principle of double entry dates from the fifteenth century and is based on the premise that eveiy transaction involves a giver and a receiver of value. Double entiy requires that each transaction be entered into two accounts, the convention being that the account of the giver is credited and the account of the receiver is debited with the same amount of money, as noted in the journal. For convenience, each account is divided centrally, and the debit items are entered on the right-hand side. It is also usual to provide a cross-reference to the journ entiy so that errors and omissions can be checked. 

We take this opportunity to thank users of the first edition who pointed out errors and omissions, or otherwise suggested improvements or additional material that should be included. We are indebted to Mrs S.Schenk who emerged from retirement to type this manuscript. 

Although considerable effort has been made to present this informatioh as accurately as possible, mistakes and errors in transcription and translation do occur. Therefore, the authors encourage the readers to consult original sources, when possible, and to feel free to point out errors and omissions of important works so that corrections and additions can be listed in the next volume. The interpretations of data and opinions expressed are often those of the authors and are not necessarily those nor the responsibility of officials of ARRADCOM or the Department of the Army... 

Thorough training, including realistic practical exercises, with participation of outside services, is advisable. A full-scale rehearsal at least once a year is recommended. (Modifications can subsequently be introduced to cater for reported errors and omissions.)... 

Pure paper-based data collection systems are inefficient in that they require large data editing overhead. Personnel at the central location must perform visual inspections of forms, compare them against the shipping log, convey submission errors and omissions to the participating sites, visually edit the major identifiers, visually inspect for completion and legibility, log the received forms, and send the received forms that pass visual inspections to data entry... 

I have been fortunate in having been able to impose upon a number of expert friends and acquaintances to read certain relevant chapters of the book. They have given invaluable help in enabling me to correct blatant inaccuracies and to avoid the inclusion of descriptions of wholly obsolete practice. I do, however, accept full responsibility for any remaining errors and omissions in the text which follows.

If a bombing scene is divided into an excessive number of zones for evidence recovery, a correspondingly excessive amount of documentation will have to be generated and maintained, with aU the attendant risks of errors and omissions. 

Despite my best intentions, I am aware of the deficiencies and limitations that still exist in this volume. I have tried to correct the numerous minor errors in the first edition which undoubtedly reflected my lax proof reading. In this respect I wish to express my sincere thanks to the numerous friends and colleagues who took the time to comment on the strengths and deficiencies of the first edition. Hopefully, the second edition has remedied the more glaring errors and omissions. In producing the second edition, my intention has been to keep the volume as concise as possible without compromising on the accuracy of the contents. 

My colleagues Drs Ted Dinan and Veronica O Keane of the Department of Psychiatry, Trinity College, Dublin, kindly offered to read critically the penultimate draft of the text. Their contribution was crucial in highlighting the errors, inconsistencies and lack of clarity in the draft. Their time and energy in helping to improve the text is gratefully acknowledged. Any errors and omissions that remain are, of course, entirely the responsibility of the author ... 

Much of the information in this chapter comes from Philip J. Rodacy of Sandia National Laboratories, Albuquerque, New Mexico, who was invited to write this chapter but was prevented by a series of circumstances beyond our control. Nevertheless, the author takes full responsibility for errors and omissions.

Reports detailing the chemistry and syntheses of thiophenes, benzothiophenes, and related ring systems that have appeared during the past year are the primary focus of this review. The synthesis of heterocycles including thiophenes has been reviewed <99JCS(P1)2849, 99JH1469>. As always, the author apologizes in advance for all errors and omissions. 

The mention of specific companies or of certain manufacturers products does not imply that they are endorsed or recommended by the World Health Organization in preference to others of a similar nature that are not mentioned. Errors and omissions excepted, the names of proprietary products are distinguished by initial capital letters.

Internal data review is the most effective mechanism for avoiding errors of interpretation and procedure. A three-tiered approach assures that errors and omissions made in Tier 1 Review are detected and corrected in Tier 2 Review. A broad Tier 3 Review often identifies errors and inconsistencies between different analyses that cannot be detected during Tier 1 or 2 Reviews of individual analyses. These errors and inconsistencies can be thus corrected and reconciled before the final report is delivered to the client. 

There also were problems with text processors in that they did not have a wide enough range of special symbols for scientific usage. The highest quality daisy-wheel printers were limited to 96 characters and often could not stop in the middle of a manuscript to change print wheels. Thus one was required to draw these special characters in the manuscript manually. This of course was another source for error and omission in preliminary drafts. Finally, the special symbols that were available were generally printer dependent, making it difficult for someone with a different printer to obtain the same symbols.
