Notes should be taken at this meeting on how to shut down the plant under any set of circumstances. Startup procedures should also be documented to see that they can be done easily and safely. It is reviews like this that help prevent the multimillion-dollar fires and explosions that have destroyed many plants and taken many lives in recent years.
