Once the computer system has been delivered to the pharmaceutical manufacturer s site and is installed and connected through field cabling and tubing to instrumentation (and possibly other systems) it is ready for site acceptance testing—this for both critical and noncritical parameters and functions. The in situ acceptance testing of the system under the SAT is a key element of engineering commissioning. For continuity, SAT test results should be analyzed and compared to the FAT results. 

In some instances elements of IQ and OQ may be executed in conjunction with, or as part of, SAT and the associated project inspection and commissioning activities (see Fig. 3). Alternatively, IQ and OQ will commence after SAT and engineering commissioning is complete. 

Operation qualification involves a high degree of dynamic testing of the computer system in conjunction with the controlled process. It normally uses an alternative medium to represent process conditions, and can be performed in conjunction with plant and equipment engineering commissioning. The Federal Trade Commission has aimounced a rule, effective November 30, 1995, that sets test procedures and labeling standards for recycled oil used as engine lubricating oil (35). The test procedures used are those contained in the Engine Oil Licensing and Certification System of the American Petroleum Institute (API) (36). The rule states in effect that if recycled oils meet the requirements of the API Certification System, such oils ate substantially equivalent to new oil for use as engine oil. This federal rule preempts certain state recycled oil rules (35). 

Follow- Up. Even after the system is deHvered, additional work is usually needed. In conventional software engineering this is viewed as software maintenance or upgrade. In the application described here, a simulation study was performed to develop the ideal control strategy. Despite the lack of interest in human factors issues in the CPI in the past, the situation is now changing. In 1985, Trevor Kletz published his landmark book on human error in the CPI An Engineer s View of Human Error (revised in 1991). Several other books by the same author e.g., Kletz (1994b) have also addressed the issue of human factors in case studies. Two other publications have also been concerned specifically with human factors in the process industry Lorenzo (1990) was commissioned by the Chemical Manufacturers Association in the USA, and Mill (1992), published by the U.K. Institution of Chemical Engineers. In 1992, CCPS and other organizations sponsored a conference on Human Factors and Human Reliability in Process Safety (CCPS, 1992c). This was further evidence of the growing interest in the topic within the CPI. 

This classification underscores the inadequacy of the approach common in reliability engineering of simply classifying errors into omission and commission categories.
