Drug dependency

It is important to note that both the quality and quantity of biological response obtained with a given drug depends very much on the assay system used to make the measurement. If the assay does not have the means to detect a given efficacy, then none will be observed this should not be taken to mean that the drug does not have that particular efficacy. A common case in point... 

The expected outcomes of the patient may include a relief of pain, an understanding of the use of die patient-controlled analgesia device (when applicable), an absence of injury, an adequate nutrition intake, an absence of drug dependence, and an understanding of and compliance with the prescribed treatment regimen. 

When a patient does not have a painful terminal illness, drug dependence must be avoided. Signs of drug dependence include occurrence of withdrawal symptoms (acute abstinence syndrome) when tiie narcotic is discontinued, requests for tiie narcotic at frequent intervals around tiie clock, personality changes if the narcotic is not given immediately, and constant complaints of pain and failure of tiie narcotic to relieve pain. Although these behaviors can have other causes, the nurse should consider drug dependence and discuss the problem with tiie primary health care provider. Specific symptoms of tiie abstinence syndrome are listed in Display 19-3. 

Each time the patient visits the outpatient clinic, the nurse evaluates the patient s response to therapy and looks for any signs that drug dependency might again be a problem. 

The care of a patient receiving a cholinergic drug depends on the drug used, the reason for administration, and the patient s response to the drug. 

Assessment of the patient receiving a sedative or hypnotic drug depends on die reason for administration and whether the drug is given routinely or as needed.

The rate of polymer erosion in the presence of incorporated anhydride and release of an incorporated drug depends on the pK of the diacid formed by hydrolysis of the anhydride and its concentration in the matrix (20). This dependence is shown in Fig. 7 for 2,3-pyridine dicarboxylic anhydride and for phthaUc anhydride. In this study, methylene blue was used as a marker. The methylene blue release rate depends both on the pK and on the concentration of diacid hydrolysis product in the matrix. However, at anhydride concentrations greater than 2 wt%, the erosion rate reaches a limiting value and further increases in anhydride concentration have no effect on the rate of polymer hydrolysis. Presumably at that point Vj, the rate of water intrusion into the matrix, becomes rate limiting.
