The following types of information are collected in most CPI safety-related data collection systems ... [Pg.260]

From this data, the problems/deviations in the operational process are selected (this means collecting not only directly safety related data). Subsequently, precursors are derived from these individual problems and deviations, according to the precursor criteria. From the previous Chapter and the refinements in this Chapter, the following criteria were retrieved to identify a precursor ... [Pg.96]

Patient safety-related data plays a pivotal role in decisions concerning patient safety in the health care system. The effectiveness of such decisions depends on the quality of patient safety data (i.e., poor quality data will lead to ineffective or poor decisions). This means that careful attention is required when collecting and analyzing such data. Nonetheless, there are many good sources for collecting various types of patient safety-related data. [Pg.165]

Safety-related data collected during clinical trials can be considered at three levels the extent of exposure common, serious, and other significant adverse events and common laboratory tests. [Pg.103]

There are various types of aviation recording and reporting systems used throughout the world. In the United States, there are four major organizations that collect and analyze aviation safety-related data. These organizations are the Federal Aviation Administration (FAA), the National Transportation Safety Board (NTSB), the National Aeronautics and Space Administration (NASA), and the Research and Special Programs Administration (RSPA) [6]. Nonetheless, some of the data systems that can be useful in obtaining aviation safety-related information are as follows [6]. [Pg.151]

The NPRDS collects failure data on safety-related systems and components. At present, more than 60 plants are reporting data. The data are compiled and disseminated in periodic report,s to the participants of the program and other potential users. In addition, special searches of tl may be requested by the... [Pg.154]

There are many scales that have been developed to measure safety-related variables. The majority of these focus on aspects of safety climate. It is not the intention of this chapter to examine these measures. Rather, the specific focus is on the factors which are direcdy related to new employee safety. Thus, the measures discussed in this chapter are restricted to those which measure attitudes and expectations which new employees bring to the workplace worker attitudes and behaviors which are particularly important for new employee adaption and behaviors, such as helping, which are associated with being a new employee. It is the opinion of this author that measurement provides evidence which can be presented to new employees, coworkers, and management in order to help explain the safety issues associated with new employees. Furthermore, the collection of data provides a degree of precision in terms of the issues faced by a specific organization, for a specific job, and related to the type of new employees being recruited. [Pg.125]

Safety-related events that enter the CSA SMS through the FMCSA data collections system (MCMIS) are placed into one of seven B ASICs (Behavior Analysis and Safety Improvement Categories). These BASICs are compliance areas, and are based on the FMCSA regulations (with the exception of the Crash BASIC, which simply tracks a carrier s crashes). The BASICs (and related regulatory Parts) are ... [Pg.557]

In a naturalistic truck driver study conducted for the FMCSA by the Virginia Tech Transportation Institute (VTTI), distraction was formd to be a significant factor in safety-related events. The data was collected using in-cab surveillance cameras that recorded the activities of 100 drivers and their surroundings for 18 months. The study involved over 735,000 miles of driving. [Pg.855]

Collect data on the prevalence and individual details of patient safety-related errors. [Pg.166]

Reliability data from safety related components in nuclear power plants have been collected since the mid-70s in France, USA and Sweden. All plant owners in the Nordic countries have joined together and established a database for component failures, which is continuously updated. A new edition of the database with updated reliability data (bodi plant specific and general) including analysis of failure rates, uncertainties etc. is published about every four years to assist the utilities in performing their PSAs. The same work has been performed with respect to initiating events. Recently, die 2nd edition has been issued, based on Nordic experience. [Pg.24]

The data collection is established only for some safety related tystems and equipments. [Pg.25]

Learn from safety-compromising incidents. It is vital that organizations learn from any safety-compromising incidents that do occur. Organizations should therefore collect and analyze data related to such incidents. [Pg.386]

The safety-related lO data of a safe node is collected in the safety payload data unit (PDU), and the data type coding corresponds to PROFINET lO. In the case of a few safety-related 10 data up to 12 bytes the 24 bit CRC2 option shall be chosen for performance reasons. There are modular devices, beside compact devices, with safety and standard I/O units and sub-addresses. A PROFINET lO head station (Device Access Point) is considered to be part of the Black Channel and is used to agree upon the structure of a PROFINET 10 message with several safety containers via start-up parameterization. One safety container corresponds to one subslot in PROFINET lO. [Pg.72]

Since 1985, lOGP has been collecting safety incident-related data from its member companies, and in 1987, it published a first comprehensive collection on global safety performance indicators [4-6]. lOGP performance indicators... [Pg.136]

Local precipitation in the form of rain, snow or ice can have a direct bearing on the design of safety related structures, particularly roofs, external cables and towers. The following data are usually collected or extrapolated ... [Pg.42]

Safety-related events that enter the SMS through the data collection system are placed into one of the seven BASICs. [Pg.149]

The fact that the model connecting error types with their causes may change as a result of gaining further experience with the data collection system means that the informahon gathered on the PIFs in a situation may also change. For example, if incident data indicates the neglect of safety procedures because of production pressures, then the questions relating to this area wUl need to be extended. [Pg.265]
