The Hull site produces organic acids and derivatives. As part of an initiative launched by the UK Health Safety Executive the site developed a number of Process Safety Performance Indicators (PSPl) in accordance with the Health Safety Executives guidance document HSG 254, which describes a process for the development of PSPI at an individual plant level. This document describes how that activity was undertaken and the subsequent learning s from the process together with the planned wider rollout.

The value of having a coherent set of safety performance indicators is that it enables regulators and industry to detect problematic operatious aud iudustrywide or systemic risks and proactively address these vulnerabilities instead of waiting for the next big accident. A good example is provided by the Norwegian Petroleum Safety Authority (PSA), which continuously collects, reviews, aud reports on performance of operations in its sector of the North Sea and works with industry and labor on improvements, as discussed in several other chapters. ...

MMS, other federal regulators, and the offshore oil and gas industry failed to develop a cohereut set of safety performance indicators and thereby failed to systematically collect and use iuforma-tiou that could have fueled a continuous learning process, which would iu turn have... 

Different definitions can have different purposes. For instance, OECD (2003) defines a safety performance indicator as a means for measuring the ehanges over time in the level of safety (related to ehemieal aecident prevention, preparedness and response), as the results of actions taken . Skogdalen et al. (2010, p 109) similarly consider a safety performance indicator to be a means for measuring the changes in the level of safety (related to major accidentprevention, preparedness and response), as a result of actions taken . These definitions reflect that they are made for comparisons over time to demonstrate developments as results of actions. Accordingly, they are not made for comparison between countries and they do not give attention to the needs of special users of the indicators. 

The quality of indicators or performance data has been an issue at stake. According to Fleming (2010), good indicators are accurate (with direct relationship to system status, and difficult to manipulate), productive (related to future system states and performance) and current (giving real-time information). Skogdalen et al. (2010, 121-122) refer to the work of the American Petroleum Institute (API) about developing Process Safety Performance Indicators for the Refining and Petrochemical Industries, and finds that the success of indicators is related to the extent to which they are ...
