Antibiotics cream/ointment

It was soon found that tyrothricin causes severe hemolysis when administered parenterally and was destroyed when given orally. The antibiotic complex or individual components are effective topically and are used in various cream, ointment, lotion and solution preparations alone or in combination with other antibiotics or topical steroids3. 

These are common when using antibiotic creams or ointments in the post-peel period between 5% and 10% of allergies should be expected when using antibiotic creams containing neomycin after a peel. The temporary thinning of the stratum corneum allows products applied on the skin after peels to penetrate more easily, which promotes the development of contact allergies. 

When a chemical blepharoplasty is performed without occlusion, the increased depth of action of the phenol sometimes translates into a persistent moist scab in the inside corner of the upper eyelid (Figure 36.16), where the phenol has macerated more intensely. Applying an antibiotic cream or ointment remedies the problem, and it should resolve before the 15th day. There are no sequelae from this slow healing. If the scab persists for more than 2 weeks, the doctor should remain alert and monitor the patient more closely. 

Conventional care often includes the application of an antibiotic cream or ointment. This local antibiotic carmot be applied in superimposed layers, and patients should wash their skin before applying a new layer of antibiotic. Cleaning the skin is a problem in itself, as it may injure the extremely sensitive skin and is often painful. As a result, patients do not always clean their skin before each new application, and put one layer of ointment on top of another. After a few days of maceration, it is almost inevitable that a secondary infection will develop. 

Neomycin currently is available in many brands of creams, ointments, and other products alone and in combination with polymyxin, bacitracin, other antibiotics and a variety of corticosteroids. There is no evidence that these topical preparations shorten the time required for healing of wounds or that those containing a steroid are more effective. 

Neomycin is an aminoglycoside antibiotic which acts as a polycationic amphiphile. It is used in topical creams, ointments, lotions, eye preparations and eardrops, and as an antibacterial preservative agent in some vaccines. Neomycin exhibits exceptional activity against Gram-negative bacteria and also has partial activity against Grampositive bacteria [72],...

Neomycin is available in numerous topical formulations, both alone and in combination with polymyxin, bacitracin, and other antibiotics. It is also available as a sterile powder for topical use. Gentamicin is available as an ointment or cream.

Mupirocin is a naturally occurring antibiotic available as a cream or ointment, and is unusual in its origin and mechanism of action. It inhibits protein synthesis, actively preventing the incorporation... 

The more general use of the antibiotic (it is now also available in creams and ointments) has led to an observed increase in resistance. Although some attempts have been made to limit its usage [75], the low incidence of resistance encountered initially is not being maintained. 

Gentamicin is absorbed slowly when it is applied topically in an ointment and somewhat more rapidly when it is applied as a cream. When the antibiotic is applied to large areas of denuded body surface (e.g., in bum patients), plasma concentrations can reach 4 pg/mL, and 2-5% of the drug may appear in the urine. 

Following a combined microdermabrasion and chemical peehng procedure, Kisner recommends applying antibiotic ointment and cold compresses to the skin. I rarely use antibiotic ointments. Instead, a bland cleanser and moisturizer such as CeraVe or Cetaphil are used for 7 days. If the patient has irritation, hydrocortisone cream 2 /2% can be applied daily, sunscreens are worn daily. In their pilot study, Hexsel et al. washed the skin with running water 4 h after retinoid acid apphcation and prescribed 1% hydrocortisone cream for the following 3 days [7]. This was combined with thrice daily application of SPF 15 sun cream for 1 week.

Important Treatment Modalities. Common ways of dealing with dermatological problems are topical treatments (such as ointments and creams) and oral treatments (drugs taken by mouth). Any bodily injury, irritation, or trauma that eliminates water, lipids, or protein from the epidermis compromises its function. Restoration of the normal epidermal barrier can often be accomplished using mild soaps and emollient creams or lotions. The often-cited dermatologic adage is If it is dry, wet it if it is wet, dry it. Consequently, wet compresses are a frequendy used remedy. A multitude of other topical treatments are available, from antibiotic, antiviral, or steroid ointments applied to treat infectious diseases or eczema to vitamin D derivative creams for psoriasis and retinoid creams for acne. Drugs can also be taken orally to treat a variety of conditions such as acne and autoimmune disorders.
