As an alternative to using synthetic analogs for disease treatment, the natural ligand for the retinoic acid receptor, all-irans-retinoic acid (ATRA), is used effectively in the treatment of acute promyelocytic leukemia (APL) (73). By comparison, acute promyelocytic leukemia is characterized by translocations involving the RARA gene (Table 39 3). These translocations result in a block of myeloid differentiation at the promyelocytic stage. It has been shown that the maturational arrest is due in part to the recruitment of the nuclear co-repressor/histone deacetylase complex.
