Some patients may experience hearing loss, which may accompany diabetes. Usually, type 2 diabetes is described in individuals with MELAS, although type 1 or insulin-dependent diabetes also may be observed. Palpitations and shortness of breath may be present in some patients with MELAS secondary to cardiac conduction abnormalities such as Wolff-Parkinson-White syndrome. Acute onset of gastrointestinal manifestations (e.g., acute onset of abdominal pain) may reflect pancreatitis, ischemic colitis, and intestinal obstruction. Numbness, tingling sensation, and pain in the extremities can be manifestations of peripheral neuropathy. Some patients may have the presentation of Leigh syndrome (i.e., subacute necrotizing encephalopathy).

The Wolff rearrangement of anti- and syn- dehydronorbomyl compounds 69 in methanol resulted in the formation of unexpected rearranged products 70-72. The photolysis of 69-anti gave five isomers viz. 70 -anti-exo, 10-anti-endo and the unexpected products l -anti-endo, l -anti-exo and 12-anti. The diazo ketone 69-syn led to two abnormal Wolff products 71 and 72 in addition to the expected Wolff rearrangement product.

The main hematologic change is acanthocytosis, a red cell abnormality (see figure 2) which has been used by some authors as a name for the disease. It is uniformly present in a-jS-lipoproteinemia and can be seen upon inspection of a wet blood smear. In cases where anisocytosis is suspected initially rather than acanthocytosis, other findings such as the low serum cholesterol level aid in differentiation. Over 50 per cent (Wolff 1965) and up to 80 per cent (Sobrevilla et al. 1964) of red cells show the typical changes they resemble thorn-apples with spinelike processes of various sizes that impart a crenated appearance. Bone-marrow examinations show the reticulocytes to be normal. The lack of rouleaux formation may contribute to the low red cell sedimentation rate (about 1 mm in the first hour) or else may result from a common cause. 

In most cases the concentration of a-lipoprotein (HDLP) was also found to be low (Salt et al. 1960 Wolff and Baumann 1961). This seems to be a nonspecific abnormality since it also occurs in steatorrhea resulting from other causes. The lipid pattern of a-lipoproteins (phospholipid/cholesterol ratio 53 26) is reflected in that of total plasma lipids (Foesyth et al. 1965). The lipid phosphorus and cholesterol content of the fraction with d > 1.063 is about normal (Ways et al. 1963). The cause for the lowered a-lipoprotein concentration is not known. A possibility is decreased formation from lipoproteins of lower density (chylomicrons and / -lipoproteins), due to the decreased concentration or absence of the latter.

Finally, studies in the UV-sensitive XP cells also showed a divergence between SCE and chromosome aberrations. XP is abnormally sensitive to induction of chromosome aberrations by certain chemicals such as 4-nitroquinoline-1-oxide (4-NQO), whereas other chemicals such as A -methyl--nitro-A -nitrosoguanidine (MNNG) and ethyl methanesulfonate (EMS) produce the same amount of chromosome aberration in XP and normal cells. Wolff-Parkinson-White (WPW) syndrome is a common type of preexcitation syndrome, an abnormal condition in which electrical impulses enter the ventricles from the atria using an accessory pathway that bypasses the atrioventricular (AV) junction. This results in a short PR intenral and a wide QRS complex with an initial slurring of the upward slope of the QRS complex, called a delta wave. Because the delta wave prolongs the QRS complex, it may be confused with a bundle-branch block.
