Speech therapist

Lifestyle modifications should be started early and continued throughout treatment because they may improve ADL, gait, balance, and mental health. The most common interventions include maintaining good nutrition, physical condition, and social interactions. Patients should avoid medications that block central dopamine, as they may worsen PD.1,18 A multidisciplinary approach using the expertise of nutritionists, speech therapists, physical therapists, occupational therapists, and social workers may optimize care but may not be covered by insurance. Patients should maintain regular visits with their optometrist or ophthalmologist and their dentist. The dentist should be informed that the patient has PD, as PD medications that decrease saliva flow may increase the risk of dental caries. 

Speech therapy There is insufficient evidence to support or refute possible benefits of speech therapy after stroke for either aphasia (Greener et al. 2002) or speech apraxia (West et al. 2005), although patients and carers value such input and stroke-care guidelines recommend speech therapy. Speech therapists also have a role in the management of dysarthria and swallowing. 

A unique complication of tube feedings in children, especially in the first year of life, is the development of feeding disorders due to oral hypersensitivity, poor oral/motor skills, and food aversion. In these children, transitioning from tube to oral nutrition is often difficult and protracted. The involvement of an occupational or speech therapist, behavioral psychologist, or other trained individual, as well as perseverance by the family often is necessary to improve oral intake. Avoidance of a strict nothing by mouth (NPO) status, if possible, and oral stimulation programs for those children who must remain NPO are recommended to avoid this complication. " ... 

A patient, who got crushed medicines because of swallowing difficulties, was referred to a speech therapist at the request of a new nursing home doctor. The speech therapist investigated the patient s swallowing and noticed that the swallowing function was all right but that the patient was afraid of tablets getting stuck in the throat. The fear was overcome by exercises and the medicines did not needed to be crushed anymore. 

Speech therapy is a method of improving speech and communication skills by altering physical, cognitive, and emotional behaviors and environments that influence the vocal tract during speech. The field has many specializations. Not aU speech therapists work with people who have communication disorders. Some work with actors and singers to improve the quality and strength of their voices. Speech therapists, especially within the United States, are required to have a master s degree in the field. 

The American Speech-Language-Hearing Association was founded in 1925. However, at the time, most American speech therapists were interested in communication disorders that dealt with aphasia, articulation, and fluency there was little interest in disorders of the voice and larynx. In the late 1930 s and early 1940 s, the Nazi regime forced many European phoniatricians and voice experts to emigrate to the United States. These emigrants initiated U.S. interest in voice pathology, which became a more prominent field of study in the 1980 s. 

When there is a communication issue, the speech therapist or phoniatrician needs to create an individual intervention plan. Evaluation of a person s communication skills, including sounds that are present and absent, helps determine the type of speech problem, the anatomical location of the problem, and also any communication strengths. This information can then be used as a baseline to compare against future speech. It is important to note that the absence of performing a communication skill does not mean the skill cannot he performed. 

To become a speech therapist in the United States, one needs to obtain a master s degree in the field. In many other countries, a bachelor s degree is accepted. Basic course work includes topics in child development, linguistics, psychology, physics and acoustics, physiolog y, anatomy, and neurology. Other qualifications vary by state. Many states require speech therapists to complete a minimum of four hundred hours of clinical experience, a thirty-six-week clinical fellowship, and the speech therapy Praxis exam to earn a clinical competence certificate (C.G.C.). 

Like all medical specialists, speech therapists and phoniatricians are constantly looking for new, more efficient therapies and treatments. Scientists and researchers also regularly reevaluate the effectiveness of treatments. 

At our facility, the Temple University Hospital, which is one of four HCFA Chronic Ventilator-Demonstration sites, the complex and diverse problems of PMV patients are treated by a diverse team comprises pulmonologists, respiratory nurses, nutritionists, psychologists, physical therapists, speech therapists, and a social worker (Fig. 2). This unit emphasizes rehabilitation and restoration of functional status despite requirements for prolonged ventilation. Special needs of patients that require PMV addressed in this unit include evaluation of the optimum form of ventilator support, special attention to swallowing dysfunction, impaired communication skills, psychological dysfunction, nutritional repletion, respiratory muscle and whole body reconditioning, as well as close attention to new or changing medical conditions.
