For children, heparin sodium is administered by intermittent IV infusion in a range of 60 to 80 units/kg of body weight every four hour. For a 57-lb child, calculate the range, in mL, of a heparin sodium injection containing 5000 units/mL to be administered daily. 

Valproate sodium injection is indicated as an IV alternative in patients for whom oral administration of valproate products is temporarily not feasible. 

Injection For IV use only. Administer as a 60-minute infusion (but not more than 20 mg/min) with the same frequency as the oral products. Use of valproate sodium injection for periods of more than 14 days has not been studied. Switch patients to oral valproate products as soon as it is clinically feasible. 

Conversion from oral products to injection When switching from oral products, the total daily dose of valproate sodium injection should be equivalent to the total daily dose of the oral product. Closely monitor patients receiving doses near the maximum recommended P.722... 

Acute head injuries A study evaluating the effect of IV valproate in the prevention of posttraumatic seizures in patients with acute head injuries found a higher incidence of death in valproate treatment groups compared with the IV phenytoin treatment group. Until further information is available, it seems prudent not to use valproate sodium injection in patients with acute head trauma for the prophylaxis of posttraumatic seizures.

Fosphenytoin is a prodrug, a compound that undergoes chemical conversion in the body to become the therapeutically active compound phenytoin. Cerebyx dosage will continue to be expressed in PEs. The most important requirement is that the salt possesses sufficient solubility at physiologically compatible pH values to permit incorporation into the dosage form. Buffering the solution to an appropriate pH can often enhance solubility. Salts may also be prepared in situ in the formulation. This is particularly useful when the main route of administration utilizes the parent drug form. Where the aqueous solubility of the salt is not sufficiently high, co-solvents may need to be added to enhance solubility (e.g. propylene glycol is used as the vehicle in phe-nobarbitone sodium injection). Parenteral solutions based on co-solvent vehicles normally cannot be directly injected intravenously because there is the risk of precipitation at the injection site. Arnold et al. (1977) proposed that the non-proton hydrates are formed by reactions of proton hydrates with formaldehyde, which possesses a large proton affinity, but this hypothesis was rejected when the reaction of formaldehyde with higher order proton hydrates was found to be endothermic (Fehsenfeld et al., 1978). After sodium was heated (up to 400 or 510 C) and membrane was broken at the pressures of 4.5 to 6.5 atm, sodium was injected into the steel vessel of 2.0 m volume.
