The use and cost of HIV service provision in England in 1996 was analyzed by Easterbrock et al. (1998). Standardized activity and case-severity data was collected prospectively in ten English HIV clinics. 5,440 patients attended the services during the first six months of 1996 and 5,708 patients during the second term. Cost estimates per patient-year for HIV service provision in 1996 varied from US 7,324 for asymptomatic patients to US 11,864 for symptomatic non-AIDS patients, and to US 31,758 for patients with AIDS. Easterbrock et al. (1998) concluded that different combinations of antiretroviral therapy affected the cost estimates differently.

The second type is the WAN (wide area network) installation. In this setup the LIMS take advantage of telecommunication technology to cover a great distance. The setup can also be used to connect disparate LANs together. In this configuration, the LIMS are installed on both the clients and a central server. The third type is the so-called centrally hosted thin client installation . For this setup, system administration is managed at a corporate center, where the LIMS are hosted and distributed via a WAN or the Internet with a virtual private network (VPN). The last and also the newest type is the ASP (Application Service Provision provider)-hosted installation. In this setup, the LIMS are hosted on a centrally managed server form and maintained by third-party specialists. Users access the LIMS with any Internet-connected PC with a standard Web browser. 

LIMS as rented service— The application service provision provider (ASP) is a means of obtaining access to software applications without the need to acquire expensive licenses and hardware or employ high-cost support resources [7]. The application is hosted on a third-party site with system maintenance, backup, and recovery provided by a third party. Products and services can be rented for a contract period on a fixed cost per user/per month basis. The advantages of obtaining LIMS in this fashion include reduced cost in initial investment and reduced requirement of resources for maintaining the LIMS. The continued security and integrity of the data transferred over the Internet is a major concern for this type of LIMS.

Finally, at launch and beyond, plans need to be in place for journal advertising and detailing of the product to doctors by sales representatives. As noted above, the customer base of pharmaceuticals is complex and the needs of individual customers differ. For GPs and specialists, information about the product will be via advertising, representative detailing and mailings. For NHS staff such as those working in Health Authorities or within PCOs, the information will need to be more focused on cost effectiveness and service provision. For practice nurses, the emphasis will be on educational materials and practical aspects of the product s use. Disease awareness programmes for patients will be developed where appropriate. 

Much of the ethos of community drug treatment as it has developed has been firmly grounded in harm-reduction policies. There is frequent debate as to exactly what such principles entail, and even how appropriate they are in countries at different stages of development. Those interested in the philosophy are referred to a paper by AL Ball (2007) from the department of HIV/Aids at the World Health Organization, which clearly still sees avoidance of HIV as one of the key aims of this policy agenda. What are the gaps in existing service provision and what client needs are currently unmet Who are the clients Some CDTs, for example, have become orientated towards providing services for tranx users because there are apparently few illicit drug users in their district. 

Given the difficulties which have faced multidisciplinary community teams, it seems almost a miracle that any have survived. The central issue is how these teams can be integrated into the existing service structure in such a way as to not only enhance the over-all pattern of service provision, but also to facilitate and support the work done by the team. If this is going to happen we need to be much clearer about what the needs of our client group are, and how we propose to meet them. The fact that so many teams have survived is a tribute to the adaptability and commitment of their individual members. In order to sustain their commitment the teams themselves need to receive the support and commitment of others. If we cannot learn this lesson from existing teams the future of community services looks bleak.
