Self certification

Contracts with the federal government require compliance with Executive Order Nos. 11625 and 12138 to utilize small and small disadvantaged businesses. The contractor/subcontractor must complete a copy of the representations and certifications providing self-certification of business classification under existing federal definitions. The representations and certifications also provide tax identification information required by the Internal Revenue Service. The original of the representation and certification form should be sent to regional purchasing and a copy included in the project file.

First of all the term CE mark is incorrect. It was amended in the directives to CE marking, which differentiates the suppliers self-declaration (CE) from a approval mark (VDE/TUV) Issued by an accredited European Union (EU) certification body. Furthermore, it is not accurate to refer to CE as a self-certification as it is really only a self-declaration and no more. Therefore, it will always be referred to it as CE marking, as used in the directives themselves. It is a symbol of the supplier s self-declaration and not a certification, approval, or mark of any kind. 

If you are sick and unable to go to work, you should contact your employer or supervisor as soon as you can on the first day of illness. When you go back to work, if you have been sick for up to seven days, you will have to fill in a self-certification form. After seven days you will need a medical certificate from your doctor and you must send it to work as soon as you can. If you are sick for four days or more your employer must pay you statutory sick pay (SSP), which can be paid for up to 28 weeks. If you are still sick after 28 weeks you can claim incapacity benefit. To claim this you will need a form from your employer or social security office. If you have a sickness problem, talk to your supervisor or someone at work who you trust, or telephone the local social security office. 

Driver application procedures, specifies some of the procedures that must he followed to obtain an initial or nonresident CDL, or transfer, renew, or upgrade a license. As of January 30,2012, this process in all states includes a driver applying for a new CDL or a driver renewing a CDL having to self-certify (tell the state what t e of driver he/she is) and presenting proof of having passed a physical, if required for that type of driver (in most cases, a valid medical certificate, also known as a medical card ). Some states use additional terminology in the self-certification process, such as referring to drivers as Tier 1, Interstate, I Tier 2, Excepted Interstate, etc. 

As soon as the state has the driver s self-certification information and proof of medical qualification, the medical certification information will then be entered into the driver s driving record, and subsequently the CDLIS information for the driver. It will then be visible to officers on the road and carriers whenever a MVR is run on the driver. As several states were not able to complete the necessary computer system upgrades, CDL drivers will still be required to carry their medical card rmtil January 30, 2015, even if they have their medical information on file with their state. 

Once the driver is into the system and the state has a copy of the driver s medical card on file, the drivers must repeat the self-certification and present the state driver licensing agency with a copy of the new medical card whenever they pass another physical.

SIL stands for safety integrity level. It is a measure of performance of an SIS. It is determined by PFD for SIF (SIS). There are four SIL levels represented by number, viz. SIL 1, 2, 3, 4. Higher the SIL number, the better will be the performance and lower will be PFD value. However, with an increase in SIL number, the cost and complexity of the system increases, but risk level reduces. It is worth noting that there could be individual component PFD but not SIL. SIL is only given to a system (SIS). SIL certification can be issued by the company (self-certification allowed), or other competent authority to indicate that appropriate procedure, analysis, and calculation... 

Certification Both products and processes can receive such a certification. Certification for the former is most common and is issued by an independent agency to show that the appropriate SIL calculations have been performed and analysis has been completed on a product. Self-certification, though not common, is also possible. Such certifications are used to signify that it is compatible for use within a system up to the certified SIL. As discussed earlier, FMEDA is normally used to determine the safe/unsafe and detected/undetected failure modes of a product. FMEDA is useful for calculations of safety failure fraction and PFD. Although not common, full certification of lEC 61508 is also possible for manufacturer s design and quality processes. 

Category 3 Equipment in this category is intended for use in areas in which explosive atmospheres caused hy gases, vapors, mists, or air/dust mixtures are unlikely to occur or, if they do occur, are likely to do so only infrequently and for a short period only. Equipment or protective systems need to guarantee a normal level of protection. Self-certification (internal control) is allowed in this category if manufacturers have sufficient testing facilities. The success of self-regulation depends to a large extent on the honesty and integrity of those following its regime. There is something of a parallel in the self-certification of machinery allowed under the Machinery Directive s but this requires the support of documentary evidence which can be examined.

Under the new CDL/medical card merger rule, the driver s medical card is now tied to his/her CDL. The driver must submit the medical examiner s certificate (i.e. wallet card) along with a self-certification to the state licensing office. The self-certification identifies whether the driver is engaged in 1) interstate commerce and subject to the medical card, 2) interstate under exempt transportation (e.g., farming), 3) intrastate and subject to the medical card, or 4) intrastate and not subject to the medical card. 

If the driver has identified himselfdierself as engaged in the furtherance of interstate commerce and subject to the medical requirements, the state will enter the self-certification declaration along with the data fields from the medical card into CDLIS. The information will appear on the driver s MVR.

The Competent Persons Self-Certification Scheme is aimed at those who carry out electrical installation work as the primary activity of their business. The government has approved schemes to be operated by BRE Certification Ltd, British Standards Institution, ELECSA Ltd, NICEIC Certification Services Ltd and Napit Certification Services Ltd. All the different bodies will operate the scheme to the same criteria and will be monitored by the Department for Communities and Local Government, formally called the Office of the Deputy Prime Minister. Installers of environmental technology systems must also be registered under the scheme.
