Safety performance incident analysis

Where a specific incident leading to safety, quality or production problems has occurred, the plant management may wish to perform a very focused intervention. This will be directed at identifying the direct and underlying causes of the problem, and developing an appropriate remedial strategy. The process for performing an analysis of this type is described in the incident analysis section of Chapter 6. [Pg.348]

Measurement of Safety Performance. Accident/Incident Investigation and Analysis... [Pg.92]

Safety performance begins with data. Data may originate with incidents and accidents. It may come from measurements using instruments. Data may come from studies of operations, behaviors and other sources. Data themselves have limited value. Data need conversion to information that workers, managers, executives, and others in charge of safety and health prevention find useful. Analysis helps make those conversions. [Pg.537]

The need for and usefulness of PSA as a tool for integrated safety evaluation became obvious in connection with the analysis of early safety related incidents and accidents in nuclear power plants. The early performance of a large number of PSAs and intensive research activities wiftiin areas of special complexity have resulted in a gradual development and a steadily increasing maturity within the field. [Pg.26]

Onshore and offshore oil and gas indusfry safety incident data have been collected and entered into its safety database by lOGP from its member companies since 1985. Today, this is the largest database of safety performance in the exploration and production industrial sector [2]. Thus, this section presents the data and its analysis from this database for 2012 [2]. [Pg.137]

Establish a fair analysis of the economic cost of incidents/accidents. The systematic analysis of safety and the trade-offs that it involves require a very realistic approach. Three sub-cases should accompany the risk analysis, in order to prepare for trade-offs and preserve as many as possible of the actions in this dimension of safety one sub-case calculates the cost of quality losses and damages, a second offers an assessment of the impact on the commercial image of the loss of quality and mediocre safety performance, and a third calculates the potential impact of what one is probably not going to do, due to lack of resources or due to trade-offs in favour of other priorities. [Pg.128]

Process safety improvement efforts will include performance goals that define the desired future state for the various elements of the process safety system. Examples may include 100-percent reporting of process safety incidents and near misses, 100-percent on-time completion of process safety training, and timely resolution for all hazards analysis recommendations. [Pg.58]

The lessons leamt firom the analysis of these incidents and safety reviews have been significant not only in improving FBTR performance but also as an important input for the design of future fast reactors. [Pg.145]

Operational feedback on trends, incidents, and accidents should trigger reanalysis when appropriate. Linking the assumptions throughout the document with the parts of the hazard analysis based on that assumption will assist in performing safety maintenance activities. [Pg.328]

FBTR has been fully commissioned with small core up to a power level of 13.4 MWt and the performance of all the safety related systems has been satisfactory. Large number of modifications was carried out based on experience feed back and analysis of various incidents to improve system performance. Construction, commissioning and operation of FBTR have given considerable amount of experience and confidence, which will help in its smooth and sustained operation at nominal power and will also give useful feedback for the design and commissioning of large fast reactors. [Pg.26]

Discussions of achievements with safety professionals whose oiganiza-tions had top scores did not produce any surprises. Incident investigation for hazard identification and analysis gets done best where the organization s culture includes accountability for superior performance. Here is an aggregate Mst of the conoments made in discussions with safety professionals in those entities with the best incident investigation systems ... [Pg.202]
