A typical HAZOP study report should include a brief system description, a list of drawings or equipment analyzed, the design intents, the HAZOP study tables, and a list of actions items.

Once all what-if analysis questions have been asked and answered along with all completed HAZOP studies of system components, a final report should be written to document all findings and recommendations. In the chemical industry (in the United States), this report is normally referred to as a process hazard analysis. This report is required under both OSHA and EPA regulations for facilities that handle or contain certain chemical commodities at certain defined quantity thresholds. However, when HAZOP studies and what-if analyses are used in general industry application, documentation of the results can be included in a written report along with any other system safety analyses that may have been performed (as described in previous chapters). Similar questions are applied to each the other guide words, and so on. Each time a component is studied the drawing or diagram should be marked. Not until all components have been studied can the HAZOP study be considered complete. Where errors occur on the drawing or more informations needed the drawing should be marked (using a different colour) and the points noted in the report.

Another qualitative approach to recognizing imd identifying possible hazards that may cause problems is a hazard and operability study (HAZOP). By identifying the goals of the study and by using guide words, a comprehensive report on the safety of a system can be created. 

Follow through on action items identified as part of the study review. Obtain resolution or close out of the recommendations. Prepare and issue any HAZOP or What-If Addendum Reports documenting recommendation resolutions or closeouts. 

Typically four stages of the HAZOP or What-If study report are provided - Preliminary, Draft, Final and Addendum. 

Like all other hazard analysis methods, reporting in HAZOP is also very important it shows the actual work carried out by the team. The purpose of HAZOP documentation is to record and represent the discussions hy the team during the study meeting. 

Therefore recording and report preparation can never be overestimated. It is worth noting that all the background information based on which study has been carried out should be part of the dossier. This will be helpful for further reference as well as to mark the changes in the future. Therefore the amount of information in the HAZOP final report will vary from case to case. Each section shall be covered fully with a selection of guided words and a heading.
