Management should acknowledge the risk results of the process hazard analysis reports. If the risks of the process hazards analysis are not acknowledged by management, review team members will feel their effort has be in vain and that recommendations do not have to be dealt with. Where management does not acknowledge their results, their importance will suffer and therefor the quality will degrade. Eventually this could produce a situation that existed before the reviews were conducted, i.e. hazards and risks are not really known or fully understood. 

As with most of a company s information where proprietary, trade secrets, or a facilities security may be involved, process hazard analysis reports may be considered confidential information. Release outside the company should be discussed with the legal staff or by the contractor agreements made with outside personnel participating in the study. A suitable distinction should be applied to the cover of any review produced documents whenever the confidentiality requirement is required.

Most of those who read a hazards analysis report are interested only in the findings and recommendations. Therefore, it makes sense to ensme that the report associated with each recommendation is as complete and discursive as possible. Once someone has read the recommendations report, he or she will have a complete picture of the issues faced by the team. In practice, it is hard to make a recommendation too detailed. 

The identified hazard is described in this row. A perennial complaint to do with hazards analysis reports is that they are too cryptic, and that insufficient background material is provided. Therefore, it is important to provide as much detail at this point—people who read and use the register months or even years later will not have any knowledge of the discussion that led up to the creation of the finding. 

General guidance to do with report writing is provided in Chapter 20. Some specific issues with regard to a hazards analysis report are discussed below. 

The advent of inexpensive, high quality digital cameras has had a dramatic impact on the process safety business. With regard to hazards analysis reports, pictures can help illustrate the findings and issues discussed. If the team was concerned with the high failure rate of a pump seal, e.g., a picture of that seal will help everyone understand what is being described.

The subsystem hazard analysis report contains a description of the subsystem and a narrative summary of key findings that specifically address the adequacy of the controls placed on any high hazards associated with the end products, the level of residual risks that remain after controls have been applied, and recommendations for further analysis or testing. The report should also describe the techniques and methodology used in performing the analysis, including risk assessment and risk acceptance criteria. The report should also contain the hazard report worksheets used in the study. 

Hazard Condition Item 2 Loss of Hydraulic Pressure. A small leak in the hydraulic cylinder system woidd result in a loss of pressure in the cylinder and a subsequent inability to maintain constant cyUnder volmne. Any damage to the cylinder jackhead would cause a leak of hydrauhc oil from the cylinder system. Such damage is possible because of an abrupt contact with the plunger mechanism and/or a failure in the neoprene seal between the jackhead and the plunger. The system safety analyst has learned from the preliminary hazard analysis report that there is no provision for oil recovery in the event of damage to the jackhead.
