The European Union enacted the directive on the control of major-accident hazards involving dangerous substances in response to the 1976 dioxin release from the ICMESA facility near Seveso, Italy the United States enacted a similar Emergency Preparedness and Community Right-to-Know Act in response to the 1984 Bhopal disaster and 1989 Phillips Pasadena, Texas, refinery explosion and fire. 

In other cases, the people may be located close to a hazard for reasons that no risk analyst could reasonably foresee. Probably the best known example of this occurred at the Texas City refinery explosion in the year 2005. Adjacent to the site of the release were temporary trailers used for project workers. All of the fatalities were to people working in those trailers (CSB, 2007). 

Senior managers are concerned primarily with long-term issues. With respect to risk, they are particularly sensitive to the potential for major environmental and safety events such as the Exxon Valdez spill, the refinery explosion at Texas City, or the Deepwater Horizon/Macondo catastrophe.

The Global Barrel addressed details of the fatahties, when it stated When a refinery tank explodes, people beyond the facility perimeter should not die or suffer significant injury. Never. At Amuay, one worker died within the refinery s area, which is an industrial tragedy. However, the remaining 40-47 of the dead should not have died no matter what sort of refinery explosion took place [18]. ... 

One of the very best all-around CSB videos is titled An Anatomy of Disaster Explosion at BP Texas City Refinery, March 28,2005 [13]. It tells the story of one of the worst industrial accidents in recent US history in words, animation and site footage. This refinery explosion arrd fire killed 15 workers, injured 180 others and resrrlted in billions of dollars in econorrtic losses. This video is nearly 56 rrrin long, but is divided into chapters. So, you can select chapters to show to certain audiences. 

Use the search term Venezuela Refinery Explosion 2012 and you will see photos of a massive refinery fire and the effects on neighbors nearby. There were reports of 39 deaths from this event.
