Periactin - Cyproheptadine

Cyproheptadine (Periactin). Cyproheptadine is an antihistamine, commonly used to alleviate allergy symptoms, that also has serotonin-blocking properties. It has been successfully used to stimulate appetite in patients with medical illnesses such as cancer or AIDS. Cyproheptadine provides a small but measurable benefit in the rapidity of weight gain during the refeeding of patients with the restricting subtype of AN. However, bulimic subtype patients apparently fare worse with the addition of cyproheptadine.

Cyproheptadine has antianaphylactic activity that is associated with its ability to slow down the release of histamine and other mediators from fat cells. It is mainly used for treating bronchial asthma attacks, allergic bronchitis, rhinitis, and allergic skin reactions as well as in adjuvant therapy for anaphylactic reactions. Synonyms of this drug are periactin and vimicon.

Many of these drugs have effects that are not mediated by Hi-receptors (Table 38.2). The antimuscarinic activity of several first-generation Hj-blockers may account for their effectiveness in combating motion sickness and their limited ability to suppress parkinsonian symptoms. There are some prophylactic treatments which are very individualized. Propanolol (p-blocker) has been used at 80-160 mg/day for up to 12 months. Methylsergide and dihydroergotamine(DHE), relatives of ergotamine, are used but have some serious side effects. Antihistamines such as cyproheptadine (Periactin ) have also been found to have tryptaminergic and anticholinergic activity in the CNS and can be used prophylactically. Calcium channel blockers have also been mentioned in an experimental vein. 

Clonidine hydrochloride (Catapres), carbamazepine (Tegretol), and methylphenidate (Ritalin) are occasionally useful in intractable cases of migraine. Cyproheptadine (Periactin) may be effective in adults with migraine it is of considerable importance in the treatment of childhood migraine, and many consider it to be the drug of first choice. Dosages range from 4 to 3 mg, three to four times a day, in adults and 4 mg, two to three times a day, in children.
