Parental cautions

Most of the childhood and young adult killers of the past, such as poliomyelitis, malaria, osteomyelitis, diphtheria, typhoid, appendicitis, and many more, have been pushed out of our minds as though they never existed. Warnings have been so overstated by those wishing to protect us, parental cautions become callous threats. Each protector endeavors to see that we do not ignore his important, unique, warning consequently leading to overstatements. [Pg.6]

Purification as their N-acetyl derivatives is satisfactory for primary, and to a limited extent secondary, amines. The base is refluxed with slightly more than one equivalent of acetic anhydride for half to one hour, cooled and poured into ice-cold water. The insoluble derivative is filtered off, dried, and recrystallised from water, ethanol, aqueous ethanol or benzene (CAUTION toxic ). The derivative can be hydrolysed to the parent amine by refluxing with 70% sulfuric acid for a half to one hour. The solution is cooled, poured onto ice, and made alkaline. It should be noted that swarf from a zirconium-titanium alloy containing approximately 50% by weight of each element is prone to pyrophoricity in air. It has also been reported that when zirconium is welded to titanium, the welded zone is much more sensitive to corrosion than either of the parent metals. In view of the highly carcinogenic nature of many nitrosamines any experiments involving their isolation must be discouraged, and for this reason, this section has only been written for completeness. This high toxicity is unfortunate because the preparation of nitrosamines from the parent amines is often facile and they provide a route to highly pure nitramines. The natural fluctuations of tics and the occasional improvement of tics on placebo mandate caution in interpreting open-label trials and anecdotal reports of alternative treatments. Antihistamines such as diphenhydramine, a mainstay of OTC sleep preparations, are also used widely by parents for their children at doses of 1 mg/kg. Most of the reports of the use of clonidine for sleep disorders are clinical and anecdotal case reports of use in children with ADFFD (Wilens et ah, 1994 Prince et ah, 1996). There are some safety concerns about using clonidine once a day at bedtime, especially in patients who take a daytime stimulant. Melatonin was studied using a double-blind, placebo-controlled, crossover design (Jan et ah, 1994) on a mixed group of 15 children with sleep disturbances, with some improvement reported. However, caution is warranted in using this agent because melatonin is unregulated, and there are concerns about the purity and safety of some commercially available preparations (Werry and Aman, 1999). [Pg.627]
