Ovarian tumors epithelial

The three major pathologic categories of ovarian tumors include sex-cord stromal, germ cell, and epithelial. About 85% to 90% of ovarian cancers are of epithelial origin. 

Epithelial ovarian tumors are composed of cells that cover the surface of the ovary, such as serous, mucinous, endometrioid, clear cell, and poorly differentiated adenocarcinomas.

While the risk that hormone replacement therapy may cause endometrial tumors has been widely discussed, less attention has been given to the possibility that it could increase the risk of epithelial ovarian cancer. Since cancer of the ovary has some risk factors in common with endometrial cancer (notably low parity and obesity), this possible risk needs to be considered, especially in view of the fact that the endometrioid epithelial type of ovarian tumor is histologically so similar to adenocarcinoma of the endometrium.

Epithelial tumors of the tube and peritubal region resemble ovarian tumors and have similar immunophenotypic features.

Wilms tumor gene product (WTl) has been established as a good marker for mesothelial cells. Studies have also demonstrated that WTl is a highly sensitive and specific marker of serous carcinomas of ovarian surface epithelial origin (both ovarian and extra-ovarian). Brenner tumors present rare ovarian tumors that occur at a mean age of 50 years. Brenner tumors constitute l%-3% of ovarian tumors. They are mostly henign, with less than 2% demonstrating borderline or malignant transformation. They are typically small, solid, unilateral ovarian tumors, with 60% of these tumors found under 2 cm in size. Extensive calcification may he observed. The vast majority is discovered incidentally in pathologic specimen of the adnexa. Brenner tumors rarely produce estrogen, and then they may be associated with endometrial thickening [87]. If cystic components are found in Brenner tumors, they may be associated with cystadenomas [88]. Up to 20% of Brenner tumors are associated with mucinous cystadenomas or other epithelial neoplasm (Fig. 9.34).
