A number of chiral 5,6-dihydropyran-2-ones and tetrahydropyran-2-ones have been prepared from key intermediates generated through an asymmetric reaction belonging to one of the following types [6]  

As in the case of dihydropyrones synthesized from compounds of the chiral pool (Section 2.3.1), the stereogenic carbon(s) created during the asymmetric reaction must not necessarily end up as C-5/C-6 of the heterocychc ring but may serve to influence the configuration of these two carbon atoms via internal induction. 

The Sharpless asymmetric epoxidations (SAEs) have been used in many cases for the synthesis of enantiopure 5,6-dihydropyrones, both in the direct mode, the conversion of a prochiral olefin into an enantioenriched epoxide, and in the kinetic resolution mode, which involves the selective epoxidation of one of the enantiomers in a racemic olefin. Other class of polyhydroxylated derivatives acting as sugar mimics consists of carbasugars i.e. sugars in which the ring-oxygen atom is replaced by the carbon atom. This area represents the essence of semi-synthesis, perhaps in its most esoteric aspect. The anticipated success of using stereochemically-defined starting materials has often entailed much experimentation to select the most appropriate compound for the task. As with other groups of natural products there are many classic examples of the use of semi-synthesis as well as more recent highly developed cases in what has come to be termed the chiron approach. 

Of the 255 patients used for the revised application, 28 (11 percent) were classified as partial responders, meaning that their tumors had shrunk by half or more, and existing lesions had not expanded in size. FDA officials were convinced by this measure of Proleukin s efficacy. They even reproduced Chiron s data in their Summary for Basis of Approval Seven of the 28 partial responders and none of the complete responders were symptomatic from their disease (PS=i) at study entry. Of these seven patients, one returned to baseline after therapy and the other six became asymptomatic (PS=o). 18 Between the ECOG status and uncertainties in how to define partial and "complete responses, proof of efficacy, now more than ever, required patient assessment and communication between physicians and patients in the clinical trial. Furthermore, the company and FDA officials had to negotiate appropriate measures of efficacy and how to scale data from various tests.
