Opium tincture/paregoric

Opioids. Activation of opioid receptors in the enteric nerve plexus results in inhibition of propulsive motor activity and enhancement of segmentation activity. This antidiarrheal effect was formerly induced by application of opium tincture (paregoric) containing morphine. Because of the CNS effects (sedation, respiratory depression, physical dependence), derivatives with peripheral actions have been developed. Whereas diphenoxylate can still produce clear CNS effects, loperamide does not Lullmann, Color Atlas of Pharmacology... 

Opioid derivatives used to treat diarrhea are listed in Table 27-3. Opium tincture (laudanum) and camphorated opium tincture (paregoric) are naturally occurring opiates that are very potent inhibitors of peristalsis. These natural agents are still available for treating diarrhea, but they have essentially been replaced by newer opioids such as diphenoxylate and loperamide. These newer opioids are somewhat less potent but may produce fewer side effects.

Opium tincture/paregoric is an opioid analgesic that enhances tone in long segments of longitudinal muscle and inhibits propulsive contraction of both circular and longitudinal muscles. Opium tincture is used in acute and nonspecific diarrhea. 

Opium tincture and camphorated opium tincture (Paregoric) have long been used as effective antidiarrheal agents. The bad taste of these liquid preparations and their abuse potential (Schedule II and III, respectively) serve to limit their use and to favor newer agents. Codeine sulfate or phosphate salt, as a single agent, is sometimes used for the short-term treatment of mild diarrhea.

Caution - Opium tincture contains 25 times more morphine than paregoric. Do not confuse opium tincture with paregoric this may lead to an overdose of morphine.

The more widely used paregoric (camphorated opium tincture) is equally effective and is frequently used in combination with other antidiarrheal agents. Codeine also has been used for short-term symptomatic treatment. 

The alkaloids derived from opium are collectively known as opiates. Morphine, codeine, and thebaine are well-known opium derivatives. Paregoric is an opium tincture (opium in an alcohol mixture). 

Papaver somniferum (opium poppy) contains a variety of opioid and related alkaloids, including codeine, morphine, noscapine, papaverine, and thebaine. Crude opium is the air-dried latex obtained by incising the unripe capsules of P. somniferum. Paregoric is ammoniated tincture of opium (Scotch paregoric) or camphorated tincture of opium (English paregoric). The use of these formulations has largely been replaced by use of the purified compounds. 

Opium. An extract of opium, containing a mixture of the total alkaloids of opium, is available as an alcoholic aqueous solution. It is available us Deudorixed Opium Tincture (IS mg/mL of morphine base) and Paregoric (2 mg/mL of morphine base). These are used primarily for the treatment of diarrhea. 

Opiates decrease intestinal motility thereby decreasing persistalsis. Constipation is a common side effect. Examples are tincture of opium, paregoric (camphorated opium tincture), and codeine. Opiates are frequently combined with other antidiar-rheal agents and can cause central nervous system (CNS) depression when taken with alcohol, sedatives, or tranquillizers. Duration of action is about 2 hours.

The use of opium dates to 4,000 b.c. At that time it was used for medicinal and recreational purposes mainly via inhalation. Today few opium-containing preparations are used, since the activity of opium is largely attributed to its morphine content. The preparations in use today are those that have constipative effects useful for the treatment of diarrhea. Preparations include pantopon, an injectable hydrochloride of opium alkaloids, and paregoric, a camphorated tincture of opium. Paregoric can be used to treat infants with opioid withdrawal signs following in utero exposure to opioids. 

Opium can be taken by mouth or it can be smoked, but since smoking was unknown in Europe and Asia until Columbus brought news of it from the New World, opium smoking did not exist before 1492. Many oral preparations of opium were made in the past. Two that survive into our own times are paregoric, a dilute tincture of opium combined with camphor, and deodorized tincture of opium, formerly known as laudanum, which is more, concentrated.

Paregoric, tincture of opium, is marketed as a 2 mg/5 mL solution and is indicated for managing both acute and chronic diarrhea. It is not widely prescribed today because of its abuse potential.

Osborne s Syrup, This is ono of the most valuable preparations that can be made for children. Simmer 11 drachms each, rhubarb root, anise seed, and liquorice root, in 45 ounces boiliu r water over a slow fire till reduced to two-thirds. Then make a syrup with 4f troy pounds white sugar, add 2 drachms each manna and compound tincture of opium (paregoric), and 225 grains salt of tartar. In warm weather, add a wineglass of French brandy. 

Velpeau s Remedy for Diarrhea and Cholera Morbus. Take 1 ouaco each tincture of opium, paregoric elixir, mid tincture of rhubarb 10 drachms essence of peppermint and 6 drachms tincture of capsicum. This is tho original receipt for this celebrated remedy. Dose for an acfult, a tea-spoonful in 1 a wine-glass sweetened water and, if required, half a dose after each loose evacuation.
