On-call duty

In 2003, the ACGME implemented new rules limiting work hours for all residents. Residents should work no more than 80 h/week or 24 consecutive hours at a time. Schedule residents for on-call duty for more than every third night. Organizations should provide them 1 day off per week. A 2008 study of pediatric residents found no change in medication errors after implementation... 

Make provision in the work schedule to allow adequate rest for those workers carrying out standby/on-call duties or overtime.

Howard and colleagues (31) suggested a reform of residents work and duty hours based on a study that assessed the levels of physiological and subjective sleepiness in 11 anesthesia residents in three conditions (1) during a normal (baseline) work schedule, (2) after an in-hospital 24-hr on-call period, and (3) after a period of extended sleep. MSLT scores were shorter in the baseline (6.7 min) and postcall (4.9 min) conditions, compared with the extended-sleep condition (12 min), and there was no significant difference between the baseline and postcall conditions. Residents daytime sleepiness on the MSLT in both baseline and postcall conditions was near or below levels associated with clinical sleep disorders, and residents were subjectively inaccurate determining EEG-defined sleep onset.

These are made up of a large number of wide, shallow blades with a very large inlet area relative to the wheel diameter. For equal duty the speeds are lower than other centrifugal fans. Emission control from heavy duty diesel engines in vehicles and stationary sources involves the use of ammonium to selectively reduce N O, from the exhaust gas. This NO removal system is called selective catalytic reduction by ammonium (NH3-SGR) and it is additionally used for the catalytic oxidation of GO and HGs.The ammonia primarily reacts in the SGR catalytic converter with NO2 to form nitrogen and water. Excess ammonia is converted to nitrogen and water on reaction with residual oxygen. As ammonia is a toxic substance, the actual reducing agent used in motor vehicle applications is urea. Urea is manufactured commercially and is both ground water compatible and chemically stable under ambient conditions [46].
