Nerve Surgery

Microsurgical techniques are fundamental for the treatment of abnormal nerve conditions such as nerve entrapments, nerve ruptures, and brachial plexus injuries. These operations involve delicate nerve repairs through direct anastomosis or the implantation of nerve grafts.

Nerves are rarely damaged during the total joint replacement surgery. However, nerve damage can occur if considerable joint deformity must be corrected in order to implant the prosthesis. With time these nerves sometimes return to normal function. 

Patients immediate post-operative pain is lower compared to a standard operation and healing and rehabiUtation more rapid. Patients can resume near-normal activities in just days. In some cases athletes, who are in prime physical condition, can return to challenging athletic activities within a few weeks. CompHcations are rare, but do occur on occasion. Most complications associated with this surgery are infection, phlebitis, excessive swelling or bleeding, blood clots, or damage to blood vessels or nerves. 

A conduction block is a type of regional anesthesia produced by injection of a local anesthetic drug into or near a nerve trunk. Examples of a conduction block include an epidural block (injection of a local anesthetic into the space surrounding the dura of the spinal cord) a trails sacral (caudal) block (injection of a local anesthetic into the epidural space at the level of the sacrococcygeal notch) and brachial plexus block (injection of a local anesdietic into the brachial plexus). Epidural, especially, and trailssacral blocks are often used in obstetrics. A brachial plexus block may be used for surgery of the arm or hand. 

Most of us have had to endure the discomfort of dentai work, be it repairing cavities or more eiaborate orai surgery. Dentists reduce the discomfort immenseiy by the use of a iocai painkiiier. Novocain, to biock the nerves in the mouth. Novocain is a weak base containing two nitrogen atoms. A reiated anaigesic, iidocaine (aiso known as Xyiocaine), is sufficientiy strong that appiying it to the skin causes the nerves in the immediate area to shut down temporariiy. 

Dysfunction may be caused by aging, systemic inflammatory diseases, a decrease in androgen hormones, surgery, ocular surface diseases (such as herpes zoster), systemic diseases, or medications that affect the efferent cholinergic nerves. Decreased tear secretion produces an inflammatory response on the ocular surface called keratoconjunctivitis sicca. This inflammation is now a target for new medications that treat dry eye.29,30... 

Denervation Loss of nerve impulse input, such as by severing a nerve during surgery or nerve impulse blockade farther up the nerve chain. 

A second surgery is required after regeneration to prevent secondary nerve injury due to compression (den Dunnen et al., 1996a). 

Inflammation. Chronic inflammatory bowel disease affecting the small bowel can lead to disturbances of intestinal motility [146], Potential mechanisms are previous surgery, development of fibrosis and strictures, malabsorption, and cross-talk between inflammatory and enteric nerves [156, 157], Patients with Crohn s disease are often included in aggregate studies of bacterial overgrowth [23, 75, 158], reflecting this link. 

The most common indication for surgery in CP is abdominal pain refractory to medical therapy. Surgical procedures that alleviate pain include a subtotal pancreatectomy, decompression of the main pancreatic duct, or interruption of the splanchnic nerves. 

In skeletal muscle fibers whose motor nerve has been severed, ACh receptors spread in a few days over the entire cell membrane. In this case, succinylcholine would evoke a persistent depolarization with contracture and hyperkalemia. These effects are likely to occur in polytraumatized patients undergoing follow-up surgery.
